血清Cys-C、α1-MG、β2-MG联合检测在肝肾综合征诊断中的价值

摘　　要：目的:探讨胱抑素C(Cys-C)、α1微球蛋白(α1-MG)、β2微球蛋白(β2-MG)在肝肾综合征诊断中的价值。方法:收集临床诊断为肝硬化患者(35例,肝硬化组)、肝肾综合征患者(35例,肝肾综合征组)的病历资料,以35例健康者作为对照。运用受试者工作特征(ROC)曲线比较肝肾综合征组、肝硬化组患者Cys-C、α1-MG、β2-MG、肌酐(Cre)及尿素氮(BUN)水平的差异,分析Cys-C、α1-MG、β2-MG与肝肾综合征的发病风险以及三者联合检测在肝肾综合征诊断中的准确性。结果:肝肾综合征组、肝硬化组的Cys-C、α1-MG、β2-MG明显高于对照组(P<0.05),Cys-C、α1-MG、β2-MG三者联合检测曲线下面积AUC(0.913)最高,诊断效能最好,同时具有较高敏感性(91.43%)、特异度(82.86%)、阳性预测值(84.20%)、阴性预测值(90.50%)及阴性似然比(0.1)。结论:Cys-C、α1-MG、β2-MG三者联合检测对肝肾综合征诊断具有较高的准确性,可作为预测的重要指标之一。Objective:To investigate the diagnostic value of cystatin C,α1-microglobulin,β2-microglobulin in early Hepato-Renal Syndrome.Methods:The patients were confirmed by clinical diagnosis,35 cases of cirrhosis,35 cases of Hepato-Renal Syndrome and 35 cases of Characteristics(ROC)Curve to compare the difference of Cys-C,α1-MG,β2-MG,Cre and BUN betwent Hepato-Renal Syndrome with cirrhosis,to analyze the risk of Cys-C,α1-MG,β2-MG with Hepato-Renal Syndrome and the accuracy of combined detection in Hepato-Renal Syndrome.Results:The Cys-C,α1-MG,β2-MG of Hepato-Renal Syndrome group and liver cirrhosis group were significantly higher than those of control group(P<0.05).Cys-C,α1-MG,β2-MG combined detection AUC(0.913)was highest,at the same time,it had high sensitivity(91.43%),specificity(82.86%),positive predictive value(84.20%),negative predictive value(90.50%)and negative likelihood ratio(0.1).Conclusion:The combination of the Cys-C、α1-MG、β2-MG has high accuracy in the early diagnosis of Hepato-Renal Syndrome.It can be used as one of the important indexes in early prediction of Hepato-Renal Syndrome.
