腹腔镜辅助阴式全子宫切除术治疗巨大子宫肌瘤的临床探究

摘　　要：目的探讨巨大子宫肌瘤患者行腹腔镜辅助阴式全子宫切除术治疗的临床效果。方法选取88例巨大子宫肌瘤患者,以随机数字表法分为对照组(44例,开腹全子宫切除术)和研究组(44例,腹腔镜辅助阴式全子宫切除术)。比较两组各项指标。结果研究组手术时间、术后排气时间、术后下床活动时间、住院时间均短于对照组,术中出血量少于对照组;研究组并发症总发生率低于对照组;与术前比,术后两组血清C反应蛋白、白细胞介素-6、白细胞介素-2水平均升高,而研究组低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜辅助阴式全子宫切除术应用于巨大子宫肌瘤患者可改善手术指标,降低术后并发症发生率和炎症反应。Objective To investigate the clinical effect of laparoscopic-assisted vaginal hysterectomy in patients with giant uterine fibroids.Methods A total of 88 patients with giant uterine fibroids were selected and divided into the control group(44 cases,open total hysterectomy)and the study group(44 cases,laparoscopic-assisted vaginal hysterectomy)according to the random number table method.The two groups of indicators were compared.Results The operation time,postoperative exhaust time,postoperative ambulation time,and hospital stay in the study group were all shorter than those in the control group,and the intraoperative blood loss was less than that in the control group.The incidence of postoperative complications in the study group was lower than that in the control group.Compared with before operation,the serum C-reactive protein,interleukin-6,and interleukin-2 levels in the two groups increased after operation,while the study group was lower than the control group.the differences were statistically significant(P<0.05).Conclusion Laparoscopic-assisted vaginal hysterectomy in patients with giant uterine fibroids can improve surgical indicators,reduce the incidence of postoperative complications and the severity of inflammatory reactions.
