摘 要:目的分析平均红细胞体积(MCV)及平均血红蛋白含量(MCH)在地中海贫血筛查中的临床应用价值。方法选取在我院基因确诊为妊娠期地贫的病例376例作为试验组,根据其基因型进行分类。同时选取在我院体检的正常妊娠人群300例作为对照组。两组人群均进行地贫基因检测及全血细胞检查。分析MCV、MCH在地贫筛查中的应用价值。结果地贫携带者MCV、MCH明显低于正常人群,差异有统计学意义(P<0.05),MCV单独检测的特异度、灵敏度、准确度分别为52.33%、53.99%、53.25%,MCH单独检测的特异度、灵敏度、准确度分别为60.67%、69.68%、65.68%,MCV联合MCH检测单独检测的特异度、灵敏度、准确度分别为89.67%、88.83%、89.20%。结论MCV联合MCH检测地贫具有较高的诊断效能,可作为地贫产前筛查的一线筛查指标。Objective To analyze the clinical application value of mean corpuscular volume(MCV)and mean hemoglobin content(MCH)in thalassemia screening.Methods A total of 376 cases of thalassemia during pregnancy in our hospital were selected as the experimental group and classified according to their genotypes.At the same time,300 cases of normal pregnant people who underwent physical examination in our hospital were selected as the control group.Both groups underwent thalassemia gene detection and complete blood cell examination.The application value of MCV and MCH in thalassemia screening was analyzed.Results The MCV and MCH of thalassemia carriers were significantly lower than those of the normal population,and the difference was statistically significant(P<0.05).The specificity,sensitivity and accuracy of MCV alone detection were 52.33%,53.99%and 53.25%,and the specificity,sensitivity and accuracy of MCH alone detection were 60.67%,69.68%,65.68%,the specificity,sensitivity and accuracy of MCV combined with MCH detection alone were 89.67%and 88.83%,respectively,89.20%.Conclusion MCV combined with MCH to detect thalassemia has high diagnostic performance and can be used as a first-line screening index for thalassemia screening.
