经脐单孔腹腔镜下子宫肌瘤剥除术治疗子宫肌瘤患者的临床疗效分析

摘　　要：目的 分析经脐单孔腹腔镜下子宫肌瘤剥除术治疗子宫肌瘤患者的临床疗效。方法 选取150例子宫肌瘤患者,以随机数字法分为常规组(75例,传统开腹子宫肌瘤切除术治疗)和腹腔镜组(75例,经脐单孔腹腔镜下子宫肌瘤剥除术治疗),统计对比两组患者手术指标,手术前后不同时间段(术前、术后2 h、术后12 h、术后48 h)的疼痛VAS评分,及术后并发症发生率。结果 腹腔镜组术中出血量明显少于对照组,手术时长、卧床时间、排气时间及住院时长均明显短于对照组,差异具有统计学意义(P<0.05)。两组患者术前、术后2 h的疼痛评分,差异无统计学意义(P>0.05);但腹腔镜组患者术后12 h、术后48 h的疼痛评分均显著低于常规组,差异具有统计学意义(P<0.05)。结论 在子宫肌瘤患者的治疗中,相较于传统开腹子宫肌瘤切除术治疗,选择经脐单孔腹腔镜下子宫肌瘤剥除术,可减轻对机体损伤,术后疼痛较轻,属于安全高效的手术方案。Objective To analyze the clinical efficacy of transumbilical single-port laparoscopic myomectomy in the treatment of patients with uterine fibroids.Methods A total of 150 patients with uterine fibroids were selected and randomly divided into routine group(75 cases,treated by traditional open myomectomy) and laparoscopic group(75 cases,treated with transumbilical single-port laparoscopic myomectomy).Surgical indicators of the two groups of patients were statistically compared,VAS scores of pain at different time periods before and after surgery(before operation,2 h after operation,12 h after operation,and 48 h after operation),and the incidence of postoperative complications were compared between the two groups.Results The intraoperative blood loss in the laparoscopic group was significantly less than that in the control group,and the operation time,bed rest time,exhaust time and hospitalization time were significantly shorter than those in the control group,and the differences were statistically significant(P0.05);however,the pain scores of the laparoscopic group at 12 h and 48 h after the operation were significantly lower than those of the conventional group,and the differences were statistically significant(P<0.05).Conclusion In the treatment of patients with uterine fibroids,compared with traditional open myomectomy,transumbilical single-port laparoscopic myomectomy can reduce the damage to the body and has less pain after surgery,which is a safe and efficient surgical plan.
