摘 要:目的分析不同病因慢性肝病患者糖脂代谢异常率分布情况,以探究慢性肝病与糖脂代谢之间的关联。方法回顾性分析2021年2月至2022年2月在首都医科大学附属北京地坛医院肝病科门诊就诊的2543例慢性肝病患者,检测其血清血脂及血糖指标。结果各组慢性肝病血脂异常率分别为:脂肪性84.22%、药物性81.88%、HBV感染性77.80%、免疫性75.45%、酒精性66.14%、HCV感染性64.53%。各组慢性肝病血糖异常率分别为:脂肪性32.39%、药物性24.16%、HBV感染性26.91%、免疫性24.55%、酒精性25.98%、HCV感染性16.24%。各组慢性肝病糖脂合并异常率分别为:脂肪性30.42%、药物性21.48%、HBV感染性22.51%、免疫性19.09%、酒精性23.62%、HCV感染性10.68%。结论不同病因所引起的糖脂代谢异常有一定差异,脂肪性肝病异常率最高,而HCV感染性肝病异常率最低。Objective To investigate the distribution of abnormal glucose and lipid metabolism in patients with chronic liver disease of different etiologies.Methods 2543 patients with chronic liver disease in the Outpatient Department of Hepatology,Beijing Ditan Hospital Affiliated to Capital Medical University from February,2021 to February,2022 were analyzed retrospectively.These patients’serum lipid and blood glucose indexes were measured.Results In terms of blood lipid,the abnormal rates of chronic liver disease in each group were as following:fatty 84.22%,drug 81.88%,HBV infection 77.80%,immunity 75.45%,alcoholic 66.14%and HCV infection 64.53%.In terms of blood glucose,the abnormal rates of chronic liver disease in each group were as following:fatty 32.39%,drug 24.16%,HBV infection 26.91%,immunity 24.55%,alcoholic 25.98%and HCV infection 16.24%.In terms of glycolipid complications,the abnormal rates of chronic liver diseases in each group were as following:fatty 30.42%,drug 21.48%,HBV infection 22.51%,immunity 19.09%,alcoholic 23.62%,HCV infection 10.68%.Conclusion There are certain level of differences in abnormal glucose and lipid metabolism caused by different causes.The abnormal rate of fatty liver disease is the highest,while the abnormal rate of HCV infectious liver disease is the lowest.
