摘　　要：目的探讨血清细胞角蛋白19片段抗原21-1(cytokeratin 19 fragment antigen 21-1,CYFRA21-1)与γ-谷氨酰转肽酶(gamma-glutamyl transpeptidase,GGT)的表达水平在食管癌转移患者中筛查的临床价值。方法收集2019年1月至2020年8月在解放军总医院手术治疗的食管癌患者98例、食管癌转移组60例和健康对照137例的临床资料,对患者基本信息及血清学指标进行统计分析。结果单因素统计分析显示,食管癌转移组患者的GGT和CYFRA21-1水平高于食管癌组与正常对照组,差异具有统计学意义(P<0.05);GGT与CYFRA21-1组合对食管癌转移诊断较优,ROC曲线下面积(AUC=0.73,95%CI:0.64~0.81),灵敏度为45%,特异性为93.9%,其二元Logistic回归模型的诊断准确度为74.1%。结论CYFRA21-1和GGT是较为理想的血液检测指标组合,有助于食管癌转移患者的筛查。Objective To explore the Clinical value of serum cytokeratin 19 fragment antigen 21-1(CYFRA21-1)and gamma-glutamyl transpeptidase(GGT)detections in screening of patients with esophageal cancer metastasis.Methods Clinical data of 98 patients with esophageal cancer who underwent operation were selected for the study and retrospectively analyzed.Among these patients,there were 60 patients with esophageal cancer metastasis and 137 normal controls who were from Chinese PLA General Hospital from January,2019 to August,2020.The basic information and serological indexes of the patients were collected and statistically analyzed.Results Univariate statistical analysis showed that several significant results as following:levels of GGT and CYFRA21-1 in the esophageal cancer metastasis group were higher than those of esophageal cancer group and normal control group(P<0.05);The diagnosis performance of GGT and CYFRA21-1 combination in esophageal cancer metastasis was ideal,with evaluation matrix including the area under ROC curve(AUC=0.73,95%CI:0.64-0.81),sensitivity 45%,specificity 93.9%,and diagnostic accuracy of binary logistic regression model for esophageal cancer metastasis 74.1%.Conclusion CYFRA21-1 and GGT are ideal combination of blood test indicators and they are helpful for the screening of patients with esophageal cancer metastasis.
