喉水平垂直部分切除术结合等离子刀治疗喉癌的效果分析

摘　　要：目的探讨喉水平垂直部分切除术结合等离子刀治疗喉癌的临床效果。方法抽取2016年6月至2020年6月南阳市中心医院收治的男性喉癌患者60例,按照手术方案分为观察组与对照组,每组30例。对照组采用全喉切除术治疗,观察组采用喉水平垂直部分切除术结合等离子刀治疗。比较两组术后12个月的嗓音功能和吞咽功能变化,术前和术后12个月甲状腺功能,及术后12个月生存情况。结果术后12个月,观察组嗓音功能3分者(13.33%,4/30)少于对照组(43.33%,13/30),P<0.05。术后12个月,观察组吞咽功能1级者(80.00%,24/30)多于对照组(53.33%,16/30),P<0.05。术后12个月,两组血清促甲状腺激素(TSH)水平高于术前,三碘甲状腺原氨酸(T3)、甲状腺素(T4)水平低于术前(P<0.05)。术后12个月,观察组血清TSH水平高于对照组,而T3和T4水平低于对照组(P<0.05)。观察组术后12个月生存率(96.66%,29/30)与对照组(93.33%,28/30)比较,差异未见统计学意义(P>0.05)。结论喉水平垂直部分切除术结合等离子刀治疗喉癌效果良好,且可改善患者术后嗓音功能和吞咽功能。Objective To investigate the effect of horizontal and vertical partial laryngectomy combined with plasma knife on laryngeal cancer.Methods A total of 60 male patients with laryngeal cancer who were admitted to Nanyang Central Hospital from June 2016 to June 2020 were selected,and the patients were divided into an observation group and a control group according to the operation plans,with 30 cases in each group.The control group was treated by total laryngectomy,and the observation group was treated by horizontal and vertical partial laryngectomy combined with plasma knife.The changes of voice function and swallowing function 12 months after operation,thyroid function before operation and 12 months after operation,and 12-month postoperative survival were compared between the two groups.Results The patients in the observation group who had a 12-month postoperative voice function score of 3(13.33%,4/30)were less than those in the control group(43.33%,13/40),P<0.05.There were more patients in the observation group with a level 1 swallowing function 12 months after operation(80.00%,24/30)than that in the control group(53.33%,16/30),P<0.05.Twelve months after surgery,serum thyroid stimulating hormone(TSH)levels in the two groups were higher than those before surgery,while triiodothyronine(T3)and tetraiodothyronine(T4)levels in the two groups were lower than those before surgery(P<0.05).Twelve months after operation,the serum TSH level in the observation group was higher than that in the control group,while the levels of T3 and T4 in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the 12-month postoperative survival rate between the observation group(96.66%,29/30)and the control group(93.33%,28/30),P>0.05.Conclusions Horizontal and vertical partial laryngectomy combined with plasma knife has a good effect in the treatment of laryngeal cancer,and can improve postoperative voice function and swallowing function.
