摘　　要：目的探讨芪参还五胶囊联合经颅直流电刺激对卒中后抑郁患者结合珠蛋白(Hp)、微小RNA(miR)-30a-5p的影响。方法前瞻性选取2019年10月至2020年10月沧州中西医结合医院收治的卒中后抑郁患者80例,按随机表法将其分为两组,对照组40例患者采用经颅直流电刺激治疗,观察组40例患者在对照组基础上结合芪参还五胶囊治疗。两组治疗疗程4周。比较两组治疗疗效,治疗前后抑郁自评量表(SDS)和汉密尔顿抑郁量表(HAMD)、日常生活活动能力(ADL)评分、美国国立卫生研究院卒中量表(NIHSS)评分、血清Hp和miR-30a-5p相对表达量变化及不良反应情况。结果观察组卒中后抑郁患者治疗总有效率为95.00%,高于对照组(75.00%),差异有统计学意义(P<0.05)。两组治疗后卒中后抑郁患者SDS评分和HAMD评分低于治疗前,观察组治疗后卒中后抑郁患者SDS评分和HAMD评分为(43.21±3.87)、(10.23±1.98)分,均低于对照组[(47.28±3.26)、(14.34±2.37)分],差异均有统计学意义(P<0.05)。两组治疗后卒中后抑郁患者ADL评分高于治疗前,而NIHSS评分低于治疗前,观察组治疗后卒中后抑郁患者ADL评分为(84.32±5.45)分,高于对照组[(72.46±6.98)分],而NIHSS评分为(7.82±1.59)分,低于对照组[(9.96±2.10)分],差异均有统计学意义(P<0.05)。两组治疗后卒中后抑郁患者血清Hp和miR-30a-5p相对表达量低于治疗前,观察组治疗后卒中后抑郁患者血清Hp和miR-30a-5p相对表达量为(95.87±9.97)mg/L、1.08±0.28,低于对照组[(107.84±10.49)mg/L、1.59±0.35],差异均有统计学意义(P<0.05)。两组用药期间均未发生明显不良反应。结论芪参还五胶囊联合经颅直流电刺激对卒中后抑郁患者疗效明显,明显减轻患者抑郁症状,下调Hp和miR-30a-5p表达。Objective To investigate the effects of Qishen Huanwu capsule combined with transcranial direct current stimulation on haptoglobin(Hp)and microRNA(miR)-30a-5p in patients with post-stroke depression.Methods The 80 patients with post-stroke depression in Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from October 2019 to October 2020 were randomly divided into two groups.40 patients in the control group were treated with transcranial DC stimulation,and 40 patients in the observation group were treated with Qishen Huanwu Capsule on the basis of the control group.The treatment course of the two groups was 4 weeks.The therapeutic effects of the two groups were compared,and the changes of self rating Depression Scale(SDS)and Hamilton Depression Scale(HAMD),activities of daily living(ADL)score,National Institutes of Health Stroke Scale(NIHSS)score,serum Hp and miR-30a-5p relative expression before and after treatment and adverse reactions were compared.Results The total effective rate of patients with post-stroke depression in observation group was 95.00%,which was higher than that in control group(75.00%),the difference was statistically significant(P<0.05).After treatment,the two groups SDS score and HAMD score of patients with post-stroke depression were lower than before treatment,the observation group SDS score and HAMD score of patients with post-stroke depression were(43.21±3.87),(10.23±1.98)points,which were lower than control group[(47.28±3.26),(14.34±2.37)points],the differences were statistically significant(P<0.05).After treatment,the ADL scores of patients with post-stroke depression in the two groups after treatment were higher than before treatment,while the NIHSS scores were lower than before treatment,after treatment,the ADL scores of patients with post-stroke depression in the observation group were(84.32±5.45)points,which were higher than those in the control group[(72.46±6.98)points],while the NIHSS scores were(7.82±1.59)points,which were lower than those in the cont
