摘 要:目的分析血清胸苷激酶(TK1)、环氧合酶-2(COX-2)水平与喉癌根治术预后的相关性。方法抽取2019年6月至2020年6月郑州大学第一附属医院收治的80例喉癌患者为研究对象。所有患者均接受喉癌根治术治疗,术后随访1年,记录患者术后复发情况。根据术后复发情况将患者分为预后不良组和预后良好组,检测所有患者入院时均血清TK1、COX-2水平。比较两组血清TK1、COX-2水平,并分析其与喉癌根治术治疗预后的关系。结果80例患者经术后1年随访,预后不良21例(26.25%,21/80),预后良好59例(73.75%,59/80);预后不良组血清TK1、COX-2水平高于预后良好组,差异有统计学意义(P<0.05)。点二列相关性检验结果显示,血清TK1水平与喉癌患者临床预后不良成正相关(r=0.745,P<0.001),血清COX-2水平与喉癌患者临床预后不良成正相关(r=0.670,P<0.001)。Logistic回归分析结果显示,入院时血清TK1、COX-2水平过表达均是喉癌患者预后不良的风险因子(OR>1,P<0.05)。结论血清TK1、COX-2表达水平是喉癌患者临床预后风险因子,即血清TK1、COX-2表达水平与喉癌患者临床预后呈明显负相关。Objective To observe the correlation of levels of serum thymidine kinase(TK1)and cyclooxygenase-2(COX-2)with the prognosis in patients with radical resection of laryngeal carcinoma.Methods A total of 80 patients with laryngeal carcinoma admitted to the First Affiliated Hospital of Zhengzhou University from June 2019 to June 2020 were selected as the study subjects.All patients received radical resection of laryngeal carcinoma,and they were followed up for 1 year after surgery to record the postoperative recurrence.And patients were divided into poor prognosis group and good prognosis group according to postoperative recurrence.All patients received serum TK1 and COX-2 detection at admission.And the levels of TK1 and COX-2 were compared between the two groups,moreover,the correlation of serum TK1 and COX-2 with clinical prognosis of patients with laryngeal cancer was analyzed.Results During 1-year follow-up,there were 21 cases(26.25%,21/80)with poor prognosis and 59 cases(73.75%,59/80)with good prognosis among the 80 patients.The levels of serum TK1 and COX-2 in the poor prognosis group were significantly higher than those in the good prognosis group(P<0.05).The results of point biserial correlation analysis showed that serum TK1 level was positively correlated with the poor prognosis of patients with laryngeal cancer(r=0.745,P<0.001),and serum COX-2 level was positively correlated with the poor prognosis(r=0.670,P<0.001).Results of Logistic regression analysis showed that overexpression of serum TK1 and COX-2 at admission were risk factors for poor prognosis in patients with laryngeal cancer(OR>1,P<0.05).Conclusions The expression levels of TK1 and COX-2 in serum are the risk factors for clinical poor prognosis of patients with laryngeal cancer,that is,the expression levels of TK1 and COX-2 in serum are significantly negatively correlated with the clinical prognosis of patients with laryngeal cancer.
