颏下岛状瓣与前臂游离皮瓣修复颌面部手术后软组织缺损的效果比较

摘　　要：目的比较颏下岛状瓣与前臂游离皮瓣对颌面部手术后软组织缺损的修复的效果,为临床修复提供依据。方法抽取河南科技大学第一附属医院2018年6月至2021年12月收治的126例颌面部手术后软组织缺损修复的患者进行研究。根据皮瓣移植部位分为颏下组与前臂组,每组63例,分别接受颏下岛状瓣修复与前臂游离皮瓣修复。比较两组手术效果、手术指标(手术时间、手术出血量)、术后恢复指标(组织愈合时间、语言功能恢复时间、吞咽功能恢复时间)、并发症及患者对外观的满意度。结果颏下组手术优良率(96.83%,61/63)与前臂组(92.06%,58/63)比较差异未见统计学意义(P>0.05)。两组手术时间、手术出血量、组合愈合时间、语言功能及吞咽功能恢复时间比较差异均未见统计学意义(P均>0.05)。颏下组患者对外观的满意度(96.83%,61/63)及并发症发生率(4.76%,3/63)与前臂组(93.65%,59/63;7.94%,5/63)比较差异均未见统计学意义(P均>0.05)。结论采取颏下岛状瓣与前臂游离皮瓣修复颌面部手术后软组织缺损,效果均较好,在治疗效果、手术指标、术后恢复指标、并发症及患者对外观满意度方面,两种修复方式作用相当,临床治疗时可根据患者的实际情况灵活选择。Objective To compare the effects of submental island flap and forearm free flap in soft tissue defect repair after maxillofacial surgery,and provide a basis for clinical repair.Methods A total of 126 patients with soft tissue defect repair after maxillofacial surgery in the First Affiliated Hospital of Henan University of Science and Technology from June 2018 to December 2021 were selected for research.According to the flap transplantation sites,they were divided into submental group and forearm group,with 63 cases in each group,respectively receiving submental island flap repair and forearm free flap repair.Surgery effects,surgical indexes(operation time,surgical blood loss),postoperative recovery indicators(tissue healing time,language function recovery time,swallowing function recovery time),complications,patient’s satisfaction with appearance were compared between the two groups.Results There was no significant difference in the excellent and good rate between the submental group(96.83%,61/63)and the forearm group(92.06%,58/63),P>0.05.There was no significant difference in surgical indexes,intraoperative blood loss,tissue recovery time,language function recovery time and swallow recovery time between the two groups(all P>0.05).There was no significant difference in satisfaction rate and incidence of complications between the submental group(96.83%,61/63;4.76%,3/63)and the forearm group(93.65%,59/63;7.94%,5/63),P>0.05.Conclusions In the repair of soft tissue defects after maxillofacial surgery,submental island flap and free forearm flap can achieve good results.In terms of appearance satisfaction,the two repair methods have equivalent effects.In clinical treatment,flexible choices can be made according to the actual situation of the patient.
