超声乳化联合不同手术对慢性闭角型青光眼患者眼压、前房深度的影响

摘　　要：目的探讨超声乳化联合不同手术对慢性闭角型青光眼(CACG)患者眼压、前房深度的影响。方法抽取濮阳市人民医院2019年3月至2020年2月收治的CACG患者102例(102眼),按随机数字表法分为观察组和对照组,每组51例(51眼)。观察组采用超声乳化联合房角分离术(SOAA),对照组采用超声乳化联合小梁切除术(Trab)。比较两组眼压、视力、前房深度、散光值及并发症发生率。结果术后1、3、6个月,两组眼压均较术前降低,前房深度均较术前升高,但差异未见统计学意义(P>0.05);术后1、3、6个月,观察组视力高于对照组,散光值低于对照组(P<0.05);观察组并发症发生率(3.92%,2/51)低于对照组(15.69%,8/51),P<0.05。结论超声乳化联合SOAA或Trab治疗CACG患者,均可改善眼压和前房深度,但SOAA在改善视力水平和散光值中效果更好,且SOAA术式并发症发生率低。Objective To investigate the effect of phacoemulsification combined with different operations on intraocular pressure and anterior chamber depth in patients with chronic angle-closure glaucoma(CACG).Methods A total of 102 patients(102 eyes)treated in Puyang People’s Hospital from March 2019 to February 2020 were selected and divided into observation group and control group by random number table method,with 51 cases(51 eyes)in each group.The observation group was treated by separation of atrial angle(SOAA),and the control group was treated by phacoemulsification combined with trabeculectomy(Trab).The intraocular pressure(IOP),visual acuity,anterior chamber depth,astigmatism and complication rate were compared between the two groups.Results IOP of the two groups were lower,and anterior chamber depth of the two groups were higher 1,3 and 6 months after operation,compared with these indexes before operation,but difference was not significant(P>0.05).The visual acuity of the observation group was higher than that of the control group,and astigmatism value was lower than that of control group 1,3 and 6 months after operation(P<0.05).The complication rate of the observation group was 3.92%(2/51),which was lower than 15.69%(8/51)of the control group(P<0.05).Conclusions Phacoemulsification combined with SOAA or Trab can improve IOP and anterior chamber depth in CACG patients,but SOAA is more effective in improving visual acuity level and astigmatism value,and the incidence of complications of SOAA is low.
