摘 要:目的 探究肺结节动态分析筛查早期肺癌的临床意义及危险因素。方法 纳入2020年1月至2020年12月在河南省安阳市人民医院就诊符合标准的207例肺结节患者,记录所有患者基本情况及肺结节情况(性别、肺部疾病史、肺结节个数、密度、位置等),进行二元logistic多因素回归分析,筛选危险因素。结果 (1)肺结节患者中性别及肺结节个数、边缘情况、位置、钙化有无因素使肺结节恶性风险增加,差异无统计学意义(P> 0.05)。(2)既往患有肺部疾病史使肺结节恶性风险增加1.052倍(OR=1.052,95%CI:0.089~9.535);肺结节实性成分增多使恶性风险增加0.457倍(OR=0.457,95%CI:0.197~10.801);肺结节中存在微血管使恶性风险增加6.858倍(OR=6.858,95%CI:6.273~9.995)。结论 肺结节动态分析有利于早期筛查肺癌,实现及早诊治;肺部疾病史及肺结节中实性成分增多、存在微血管是肺癌的危险因素。Objective To explore the clinical significance and risk factors of lung nodule dynamic analysis in screening early lung cancer.Methods A total of 207 patients with pulmonary nodules who met the criteria in our hospital from January 2020 to December 2020 were enrolled.The basic information and pulmonary nodules(gender,history of pulmonary disease,number of pulmonary nodules,density,location,etc.)of all patients were recorded.Risk factors were screened by binary logistic multivariate regression analysis.Results(1)There were no significant differences in gender,the number of pulmonary nodules,marginal condition,location,calcification and whether there were factors that increased the risk of malignant lung nodules(P>0.05).(2)A history of pulmonary disease was associated with a 1.052-fold increase in the risk of pulmonary nodule malignancy(OR=1.052,95%CI:0.089~9.535).The increase of solid components in pulmonary nodules increased the risk of malignancy by 0.457 times(OR=0.457,95%CI:0.197~10.801).The presence of microvessels in pulmonary nodules increased the risk of malignancy by 6.858 times(OR=6.858,95%CI:6.273~9.995).Conclusion Dynamic analysis of pulmonary nodules is beneficial to early screening of lung cancer and early diagnosis and treatment.History of pulmonary disease,increased solid components in pulmonary nodules and presence of microvessels are risk factors for lung cancer.
