尿pH值联合血清CRP对脑梗死患者痛风急性发作的预测价值

摘　　要：目的探讨尿pH值联合血清CRP对脑梗死患者痛风急性发作的预测价值。方法选择2019年1月至2021年5月台州恩泽医疗中心(集团)恩泽医院神经内科住院的脑梗死患者100例,将处于痛风急性发作期的60例设为观察组,无痛风急性发作的40例为对照组。检测两组患者的尿pH值及血清CRP并分析比较。结果观察组尿pH值低于对照组,血清CRP值高于对照组,差异均有统计学意义(均P<0.01)。尿pH值预测痛风急性发作的AUC为0.89,最佳临界值为6.25;血清CRP预测痛风急性发作的AUC为0.82,最佳临界值为5.27 mg/L。结论脑梗死合并痛风患者,尿pH值<6.25或血清CRP>5.27 mg/L,容易出现痛风急性发作,需积极预防。Objective To explore the predictive value of urine pH value combined with serum C-reactive protein(CRP)for acute gout attack in patients with cerebral infarction.Methods One hundred patients with cerebral infarction admitted in Taizhou Hospital of Zhejiang Province affiliated to Wenzhou Medical University from January 2019 to may 2021 were enrolled,including 60 patients complicated with acute gout attack(gout group)and 40 patients without acute gout(control group).The urine pH value and blood CRP of the two groups were measured.Results The urine pH value of the gout group was lower than that of the control group,while the serum CRP level was higher than that in the control group(both P<0.01).The AUC of urine pH was 0.89,and the best cut-off point was 6.25;the AUC of serum CRP level was 0.82,and the best cut-off point was 5.27 mg/L.Conclusion For cerebral infarction patients with increase uric acid level,when urine pH value is lower than 6.25,or CRP is higher than 5.27mg/L,they are likely to have acute gout attack.
