穴位埋线联合医用臭氧和甲泼尼龙琥珀酸钠不同部位注射治疗根性坐骨神经痛的临床疗效和安全性研究

摘　　要：目的研究根性坐骨神经痛患者采取穴位埋线联合医用臭氧和甲泼尼龙琥珀酸钠不同部位注射治疗根性坐骨神经痛的临床疗效和安全性。方法选择2019年4月~2021年9月期间收治的108例根性坐骨神经痛患者作为研究对象,采用随机数字表法将入选患者分为研究组与对照组,每组54例,其中对照组采取医用臭氧5 ml(30μg/ml)以及甲泼尼龙琥珀酸钠40 mg注射治疗,研究组在对照组基础上采取中医穴位埋线治疗,比较两组患者的治疗效果和安全性。结果研究组治愈率为92.59%明显较对照组治愈率74.07%提高(χ^(2)=5.469,P<0.05);治疗前,两组的疼痛评估指数(Pain rating index,PRI),包括知觉、感情以及疼痛评定指数总分三项均无显著差异(t=0.392,0.670,0.799,P>0.05)。治疗后,研究组PRI知觉、PRI感情以及PRI总分均较对照组降低(t=-8.012,-5.095,-7.493,P<0.05);治疗前,两组视觉模拟评分法(Visual analogue scale,VAS)评分、六点行为评分法(The 6-point behavioral rating scale,BRS-6)评分以及Oswestry功能障碍指数(Oswestry disability index,ODI评分)均无显著差异(t=0.636,0.903,-1.054,P>0.05),治疗后,研究组VAS、BRS-6以及ODI评分均较对照组明显降低(t=-5.943,-12.544,-29.923,P<0.05);研究组不良反应发生率3.70%与对照组发生率5.56%比较,差异无统计学意义(χ^(2)=0.392,P>0.05)。结论根性坐骨神经痛患者采取中医穴位埋线结合医用臭氧以及甲泼尼龙琥珀酸钠治疗效果显著,可明显缓解疼痛感,降低对患者伤害,改善肢体功能受限情况,促进正常生活能力恢复。Objective To investigate the clinical efficacy and safety of acupoint catgut embedding combined with medical ozone and methylprednisolone sodium succinate injected into different parts of the body in the treatment of root sciatica.Methods 108 patients with radicular sciatica admitted to our hospital from April 2019 to September 2021 were selected as the study subjects.The selected patients were divided into the study group and the control group by random number table method,54 cases in each group.The control group was treated with 5 ml(30μg/ml)medical ozone and 40 mg methylprednisolone sodium succinate.The study group was treated with catgut embedding at acupoints of traditional Chinese medicine on the basis of the control group to compare the therapeutic effects and safety of the two groups.Results The cure rate of the study group was 92.59%,which was significantly higher than 74.07%of the control group(χ^(2)=5.469,P<0.05).Before treatment,there was no significant difference in PRI perception,PRI emotion and pain assessment index(total score of PRI)between the two groups(t=0.392,0.670,0.799,P>0.05);After treatment,it was found that the total scores of PRI perception,PRI emotion and PRI in the study group were lower than those in the control group(t=-8.012,-5.095,-7.493,P<0.05).There were no significant differences in visual analogue scale(VAS),the 6-point behavioral rating scale(BRS-6)and oswestry disability index(ODI)between the two groups before treatment(t=0.636,0.903,-1.054,P>0.05);After treatment,VAS,BRS-6 and ODI scores in the study group were significantly lower than those in the control group(t=-5.943,-12.544,-29.923,P<0.05).The incidence of adverse reactions in the study group was 3.70%,which has no significant difference compared to 5.56%in the control group(χ^(2)=0.392,P>0.05).Conclusion Acupoint catgut embedding combined with medical ozone and methylprednisolone sodium succinate is effective in treating radicular sciatica,which can obviously relieve pain,reduce injury to patients,improve limb func
