中医益血汤联合粒细胞集落刺激因子对乳腺癌术后化疗患者血象、免疫功能及预后的影响

摘　　要：目的探讨中医益血汤联合粒细胞集落刺激因子(G-CSF)对乳腺癌术后化疗患者血象、免疫功能及预后的影响。方法选择2017年11月至2020年6月南昌市第三医院收治的70例乳腺癌术后化疗患者作为研究对象,采用随机数字表法分为观察组和对照组,各35例。对照组于化疗结束后采用G-CSF治疗,观察组于对照组基础上采用中医益血汤治疗。比较两组患者血象指标[白细胞(WBC)、血小板(PLT)、血红蛋白(Hb)]、免疫功能[CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)]、乳腺癌患者生命质量测定量表(FACT-B)(情感、生理、功能、家庭/社会状况、附加关注)、随访1年复发率及生存率。结果治疗后,两组患者PLT水平低于本组治疗前,且观察组PLT水平低于对照组,差异有统计学意义(P<0.05);两组患者WBC、Hb水平高于本组治疗前,且观察组WBC、Hb水平高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者CD8^(+)水平低于本组治疗前,且观察组CD8^(+)水平低于对照组,差异有统计学意义(P<0.05);两组患者CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于本组治疗前,且观察组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组,差异有统计学意义(P<0.05)。治疗后,两组患者情感、生理、功能、家庭/社会状况、附加关注评分高于本组治疗前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组复发率低于对照组,生存率高于对照组,差异有统计学意义(P<0.05)。结论中医益血汤联合G-CSF治疗乳腺癌术后化疗患者,可改善血象、免疫功能,获得较好的预后,临床应当关注。Objective To explore the effect of TCM Yixue Decoction combined with granulocyte colony stimulating factor(G-CSF)on hemogram,immune function and prognosis of patients with breast cancer after chemotherapy.Methods A total of 70 patients with postoperative chemotherapy of breast cancer admitted to the Third Hospital of Nanchang from November 2017 to June 2020 were selected as the research objects.They were divided into observation group and control group according to the random number table method,with 35 cases in each group.The control group was treated with G-CSF after chemotherapy,while the observation group was treated with TCM Yixue Decoction on the basis of the control group.The blood picture indexes(white blood cell[WBC],platelet[PLT],hemoglobin[Hb]),immune function(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)),breast cancer patients'quality of life scale(FACT-B)(emotion,physiology,function,family/social status,additional attention)were compared between the two groups.Results After treatment,the PLT level of patients of the two group was lower than that of before treatment,and the PLT level of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The WBC and Hb levels of patients of the two group were higher than thosse before treatment,and WBC and Hb levels of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).After treatment,the CD8^(+)levels of patients of the two group were lower than those before treatment,and the CD8^(+)level of the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels of patients of the two group were higher than those before treatment,and CD3^(+), CD4^(+), CD4^(+)/CD8^(+) levels of the observation group were higher than those of the control group, and the differences were statistically significant (P<0.05).
