摘  要:目的探讨人乳头状瘤病毒(HPV)感染对乳腺癌组织Ki-67、HER-2蛋白表达水平及预后的影响。方法回顾性分析2014年2月-2016年1月荆门市第一人民医院300例行改良根治术的乳腺癌患者乳腺癌组织标本及临床资料,采用流式荧光杂交法及免疫组化二步法分别检测标本中HPV感染及Ki-67、HER-2蛋白的表达,比较HPV阳性患者与阴性患者乳腺癌组织Ki-67、HER-2表达水平的差异,分析HPV感染对患者预后的影响。结果300例患者中共有101例HPV阳性患者,感染率为33.67%;发生淋巴结转移乳腺癌患者HPV感染阳性率高于未发生淋巴结转移患者(P<0.05);肿瘤直径大、分化程度低、临床分期高、淋巴结转移患者Ki-67、HER-2阳性表达率高于其他患者(P<0.05);HPV阳性组Ki-67、HER-2阳性表达率高于HPV阴性组(P<0.05);HPV阳性组5年无进展生存率78.22%(79/101)低于HPV阴性组87.44%(174/199)(P<0.05)。结论HPV感染增加乳腺癌患者Ki-67、Her-2阳性表达,增加淋巴结转移概率,降低术后5年无进展生存率。OBJECTIVE To explore the effects of human papillomavirus(HPV)infection on expression levels of Ki-67 and HER-2 proteins in breast cancer tissues and prognosis.METHODS A retrospective analysis was performed on the breast cancer tissue specimens and clinical data from 300 patients with breast cancer undergoing modified radical mastectomy in Jingmen First People’s Hospital between Feb 2014 and Jan 2016.HPV infection and the expressions of Ki-67 and HER-2 proteins in specimens were detected by flow fluorescence hybridization and two-step immunohistochemistry.The differences in expression levels of Ki-67 and HER-2 in breast cancer tissues between the HPV-positive and HPV-negative patients were compared.The effects of HPV infection on prognosis were analyzed.RESULTS Among the 300 patients,there were 101 HPV-positive cases(33.67%).The positive rate of HPV infection in the breast cancer patients with lymph node metastasis was significantly higher than that without lymph node metastasis(P<0.05).The positive expression rates of Ki-67 and HER-2 in patients with long tumor diameter,low differentiation,high clinical staging and lymph node metastasis were significantly higher than those in the other patients(P<0.05).The positive expression rates of Ki-67 and HER-2 in the HPV-positive group were significantly higher than those in the HPV-negative group(P<0.05).The 5-year progression-free survival rate in the HPV-positive group was significantly lower than that in the HPV-negative group[78.22%(79/101)vs 87.44%(174/199)](P<0.05).CONCLUSION HPV infection will increase positive expressions of Ki-67 and Her-2,increase incidence of lymph node metastasis and reduce 5-year progression-free survival rate in breast cancer patients.
