前方牵引联合螺旋扩弓器治疗骨性Ⅲ类错[牙合]畸形的疗效及对患者生活质量的改善作用

摘　　要：目的:探讨前方牵引联合螺旋扩弓器在骨性Ⅲ类错[牙合]畸形治疗中的疗效及对患者生活质量的影响。方法:选取2017年12月-2020年12月于笔者医院就诊的106例替牙期骨性Ⅲ类错[牙合]畸形患者为研究对象,随机分为单一组和联合组,各53例。单一组单纯采用前方牵引,联合组采用前方牵引联合螺旋扩弓器。比较两组临床效果、头影测量指标,并采用口腔健康影响程度量表(OHIP-14)评价两组生活质量。结果:单一组有效37例(69.81%),无效16例(30.19%),联合组有效46例(86.79%),无效7例(13.21%),联合组有效率高于单一组,差异有统计学意义(χ^(2)=4.498,P<0.05)。治疗后,两组SNA、ANB、FH-MP、U1-SN、Wits均大于治疗前,SN-PP、L1-MP均小于治疗前,且联合组ANB、FH-MP、SN-PP、L1-MP、Wits均大于单一组,U1-SN小于单一组(P<0.05)。治疗后,两组OHIP-14评分均低于治疗前,且联合组低于单一组(P<0.05)。结论:前方牵引联合螺旋扩弓器在骨性Ⅲ类错[牙合]畸形治疗中疗效确切,可改善颌骨发育,解决凹面型的不足,并提高患者的生活质量。Objective To investigate the effect of protraction combined with screw expansion in the treatment of Class Ⅲ skeletal malocclusion and its influence on quality of life.Methods Total of 106 patients with Class Ⅲ skeletal malocclusion in the mixed dentition admitted to our hospital from December 2017 to December 2020 were selected.They were randomly divided into a single group(n=53)and a combined group(n=53).The patients in the single group were treated with protraction only,and the patients in the combined group were treated with protraction combined with screw expansion.Compare the clinical effects and cephalometric indicators of the two groups,and the Oral Health Impact Scale(OHIP-14)was used to evaluate the quality of life of the two groups.Results In the single group,37 cases(69.81%)were effective,16 cases(30.19%)were ineffective,in the combined group,46 cases(86.79%)were effective,7 cases(13.21%)were ineffective,and the effective rate in the combined group was higher than that in the single group(χ^(2)=4.498,P<0.05).After treatment,the patient’s SNA,ANB,FH-MP,U1-SN,and Wits in the two groups were larger than those before treatment,and the patient’s SN-PP and L1-MP were smaller than those before treatment,and the patient’s ANB,FH-MP,SN-PP,L1-MP and Wits in the combined group were larger than the single group,U1-SN was smaller than the single group(P<0.05).After treatment,the OHIP-14 scores of the two groups were lower than before treatment,and the combined group was lower than the single group(P<0.05).Conclusion The protraction combined with screw expansion has a good effect in the treatment of Class Ⅲ skeletal malocclusion,which can improve the development of the jaw,solve the deficiencies of the concave shape,and improve the quality of life of patients.
