上颌中切牙即刻种植和位点保存延期种植的疗效及对红白美学效果的影响

摘　　要：目的:探讨即刻种植和位点保存延期种植在上颌中切牙种植患者中的疗效及对红白美学效果的影响。方法:选取2019年1月-2020年12月在笔者医院口腔科行上颌中切牙种植修复的102例患者为研究对象,根据治疗方法不同分为A组和B组,每组51例。A组采用即刻种植修复,B组采用位点保存延期种植修复。记录两组牙槽骨高度、红色美学指数(PES)、白色美学指数(WES)、种植体稳定值及并发症情况。结果:术后6个月,两组牙槽骨唇舌向、近远中向、垂直向骨高度均有不同程度的减少,但两组牙槽骨高度变化值比较,差异均无统计学意义(P>0.05)。术后6个月,A组牙龈曲线、颜色、质地及PES总分均高于B组;A组外形轮廓、表面质地及WES总分均高于B组,差异均有统计学意义(P<0.05)。术后6个月,两组种植体稳定值均大于术后即刻,但组间比较差异无统计学意义(P>0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论:即刻种植和位点保存延期种植在上颌中切牙种植患者中均具有良好疗效,但即刻种植的美学效果更佳。Objective To investigate the effect of immediate implant and site preservation delayed implant in patients with maxillary central incisor implantation and its influence on the aesthetic effect of red and white.Methods Total of 102 patients who underwent maxillary central incisor implant restoration in the Department of Stomatology of our hospital from January 2019 to December 2020 were selected and divided into group A(n=51)and group B(n=51)according to the treatment method.Group A used immediate implant restoration,and group B used site preservation and delayed implant restoration.The alveolar bone height,pink esthetic score(PES),white esthetic score(WES),implant stability and complications were recorded in the two groups.Results At 6 months after operation,the alveolar bone height of the two groups decreased in different degrees in the lip-tongue,mesiodistal,and vertical directions,but there was no significant difference in the changes of the two groups(P>0.05).At 6 months after operation,the gingival curve,color,texture and total score of PES of group A were higher than group B(P<0.05).At 6 months after operation,the contour,surface texture and total score of WES of group A were higher than group B(P<0.05).At 6 months after operation,the implant stability of the two groups was greater than immediately after operation,but there was no significant difference of the two groups(P>0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion Immediate implant and site preservation delayed implant have good effects in patients with maxillary central incisor implantation,but the aesthetic effect of immediate implant is better.
