口腔正畸联合牙周夹板与单纯牙周夹板治疗对牙周病致前牙移位患者牙齿功能及美学效果的影响

摘　　要：目的:探究口腔正畸联合牙周夹板与单纯牙周夹板治疗对牙周病致前牙移位患者牙齿功能及美学效果的影响。方法:选取2015年1月-2020年1月于笔者医院治疗的因牙周病致前牙移位的80例患者为研究对象,采用简单随机分组法将患者分为联合组和对照组,每组40例。对照组采用单纯牙周夹板治疗,联合组在对照组基础上联合正畸治疗。观察比较两组患者治疗前后各相关临床指标、牙齿松动度、血清炎性因子水平以及患者满意度。结果:治疗后,联合组探诊深度、探诊出血位点百分比、临床附着丧失和临床牙冠长度均明显低于对照组(P<0.05),牙齿松动度明显小于对照组(P<0.05);联合组患者白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)和C反应蛋白(CRP)水平均明显低于对照组(P<0.05);联合组患者功能性、舒适性和美观性评分均明显高于对照组,差异均有统计学意义(P<0.05)。结论:口腔正畸联合牙周夹板治疗牙周病致前牙移位患者矫正效果较为理想,可有效改善其临床症状,抑制炎症反应,且美学效果好,患者满意度高。Objective To investigate the effects of orthodontics combined with periodontal splint and periodontal splint alone on the dental function and aesthetic effect of anterior tooth displacement caused by periodontal disease.Methods A total of 80 patients with anterior tooth displacement due to periodontal disease treated in our hospital from January 2015 to January 2020 were selected as the research objects,and the patients were divided into combined group and control group by simple random grouping method,with 40 cases in each group.The control group was treated with periodontal splint alone,and the combined group was treated with orthodontics on the basis of the comparison group.Clinical indicators,tooth looseness,serum inflammatory factors and patient satisfaction were observed and compared between the two groups before and after treatment.Results After treatment,probing depth,percentage of probing bleeding site,clinical attachment loss and clinical crown length in the combined group were significantly lower than those in the control group(P<0.05),and tooth loosening degree was significantly lower than that in the control group(P<0.05).The levels of interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)in combined group were significantly lower than those in control group(P<0.05).The scores of function,comfort and aesthetics in combined group were significantly higher than those in control group(P<0.05).Conclusion Orthodontic treatment combined with periodontal splint for anterior tooth displacement caused by periodontal disease has ideal effect,which can effectively improve the clinical symptoms,inhibit inflammatory reaction,and has good aesthetics and high patient satisfaction.
