OCT4及其假基因POU5F1B在子宫内膜腺癌中的表达及临床意义

摘　　要：目的分析八聚体结合转录因子4(OCT4)及其假基因POU结构域5类转录因子1B(POU5F1B)在子宫内膜腺癌中的表达情况,并探讨两者在子宫内膜腺癌中的临床意义。方法纳入60例子宫内膜腺癌患者,并收集其子宫内膜腺癌组织,同时纳入50例子宫内膜良性病变患者,并收集其增生期子宫内膜组织。采用实时荧光定量PCR检测组织中的OCT4 mRNA、POU5F1B mRNA的表达水平。用免疫组织化学法检测组织中OCT4蛋白的阳性表达情况。分析OCT4蛋白阳性表达率与子宫内膜腺癌患者临床病理参数的关系。结果子宫内膜腺癌组织中POU5F1B mRNA、OCT4 mRNA的表达水平均高于增生期子宫内膜组织(均P<0.05)。子宫内膜腺癌组织中的OCT4蛋白高表达率高于增生期子宫内膜组织(P<0.05)。临床分期为Ⅱ~Ⅳ期、肌层浸润深度>1/2、有淋巴结转移的子宫内膜腺癌患者的OCT4蛋白高表达率分别高于临床分期为Ⅰ期、肌层浸润深度≤1/2、无淋巴结转移的患者(均P<0.05)。结论OCT4及其假基因POU5F1B在子宫内膜腺癌中的表达上调,并且OCT4蛋白的表达情况与患者的临床分期、浸润深度、淋巴结转移有关。假基因POU5F1B可能通过调控OCT4来影响子宫内膜腺癌的发生和发展。Objective To analyze the expressions of octamer-binding transcription factor 4(OCT4)and its pseudo-gene POU domain class 5 transcription factor 1 B(POU5 F1 B)in adenocarcinoma of endometrium,and to explore the clinical significance of the two as above in adenocarcinoma of endometrium.Methods A total of 60 patients with endometrial carcinoma were enrolled,and their tissues of endometrial adenocarcinoma were obtained;simultaneously,50 patients with endometrial benign lesion were enrolled,and their endometrial tissues in proliferative phase were obtained.The expressions of OCT4 mRNA,and POU5 F1 B mRNA in tissues were detected by the real-time fluorescent quantitative PCR.The positive expression of OCT4 protein was detected by the immunohistochemistry.The relation of positive expression rate of OCT4 protein with clinicopathological parameters of patients with endometrial adenocarcinoma was analyzed.Results The mRNA expressions of POU5 F1 B and OCT4 in tissues of endometrial adenocarcinoma were higher than those in tissues of endometrium with proliferative phase(all P<0.05).The high-expression rate of OCT4 protein in tissues of endometrial adenocarcinoma was higher than that in tissues of endometrium with proliferative phase(P<0.05).The high-expression rate of OCT4 protein in endometrial adenocarcinoma patients with clinical stagesⅡ-Ⅳ,depth of myometrial invasion>1/2,and with lymph node metastasis was higher than that in endometrial adenocarcinoma patients with clinical stageⅠ,depth of myometrial invasion≤1/2,and without lymph node metastasis(all P<0.05).Conclusion The expressions of OCT4 and its pseudo-gene POU5 F1 B are up-regulated in adenocarcinoma of endometrium,and the expression of OCT4 protein is related to clinical stage,invasive depth,and metastasis of lymph nodes.Pseudo-gene POU5 F1 B may affect the occurrence and development of endometrial adenocarcinoma through regulating OCT4.
