矢状窦旁脑膜瘤患者显微切除术后导致脑水肿相关因素分析

摘　　要：目的:分析影响矢状窦旁脑膜瘤患者显微切除术后脑水肿形成相关因素.方法:选取2019年5月至2020年8月在确山县人民医院行显微切除术治疗的86例矢状窦旁脑膜瘤患者作为研究对象.术后对其随访6个月,统计所有患者术后脑水肿发生情况.根据术后患者是否发生脑水肿将其分为发生组与未发生组.设计临床资料调查表,对两组患者临床资料相关因素进行统计学分析.结果:术后对其随访6个月,86例矢状窦旁脑膜瘤患者行显微切除术后,发生脑水肿12例,发生率为13.95%.对相关影响因素进行多因素非条件Logistic回归分析结果显示,静脉损伤、高血压史、术中脑出血量≥40mL、肿瘤直径≥4cm是导致矢状窦旁脑膜瘤患者显微切除术后发生脑水肿的危险因素(OR>1,P<0.05).结论:矢状窦旁脑膜瘤患者显微切除术后发生脑水肿可能与患者静脉损伤、高血压史、术中脑出血量≥40mL、肿瘤直径≥4cm等因素有关.Objective:To analyze the related impact factors of brain edema after microsurgical resection in patients with parasagital meningioma.Methods:Eighty-six patients with parasagttal meningioma who underwent microsurgical resection in Queshan County People's Hospital from May 2019 to August 2020 were selected as research subjects.All patients were followed up for 6 months.The occurrence of postoperative brain edema of all patients was counted,and the clinical data questionnaire was designed to find the related impact factors of brain edema after microsurgical resection in patients with parasagttal meningioma.Results:After 6 months of follow-up,brain edema occurred in 12 of 86 patients with parasagital meningioma after microsurgical resection,with an incidence of 13.95%;the results of Logistic regression analysis showed that venous injury,history of hypertension,intraoperative cerebral hemorrhage≥40mL and tumor diameter≥4cm were the risk factors of brain edema after microsurgical resection in patients with parasagittal meningioma(OR>1,P<0.05).Conclusion:Cerebral edema after microsurgical resection in patients with parasagittal meningioma may be related to the venous injury,history of hypertension,intraoperative cerebral hemorrhage≥40mL,tumor diameter≥4cm and so on.
