巨大翼状胬肉切除结合角膜缘干细胞移植术后干性羊膜覆盖对巨大翼状胬肉患者的影响

摘　　要：目的探讨分析巨大翼状胬肉切除结合角膜缘干细胞移植术后干性羊膜覆盖对巨大翼状胬肉患者的影响。方法选取2020年1月至2021年12月东莞爱尔眼科医院收治的50例巨大翼状胬肉患者,按随机数字表法分为观察组和对照组,对照组给予巨大翼状胬肉切除结合角膜缘干细胞移植术治疗,观察组联合干性羊膜覆盖治疗,比较两组患者角膜瘢痕评估,术前及术后眼部相关指标,术后当天、术后1 d、5 d及10 d VAS评分。结果两组患者术后角膜瘢痕发生率比较,差异无统计学意义(P>0.05);术前两组患者裸眼视力、角膜散光度、角膜地形图表不对称指数比较,差异无统计学意义(P>0.05);术后两组患者裸眼视力显著提高,角膜散光度、角膜地形图表不对称指数降低,且术后观察组患者裸眼视力、角膜散光度、角膜地形图表不对称指数优于对照组(P<0.05);术后当天、术后1 d、5 d及10 d两组患者VAS评分比较,差异无统计学意义(P>0.05)。结论巨大翼状胬肉切除结合角膜缘干细胞移植术后干性羊膜覆盖可显著缓解患者疼痛,改善患者裸眼视力及散光情况,促进角膜创面的愈合,值的推广。【Objective】To investigate and analyze the effect of dry amniotic membrane coverage on patients with giant pterygium after giant pterygium excision combined with limbal stem cell transplantation.【Methods】Fifty patients with giant pterygium who were treated from January 2020 to December 2021 were selected.The patients were divided into an observation group and a control group.The control group was treated with giant pterygium excision combined with limbal stem cell transplantation,and the observation group was treated with dry amniotic membrane covering.The corneal scar evaluation,preoperative and postoperative ocular correlation between the two groups were compared.Indicators,VAS score on the day after surgery,1d,5d and 10d after surgery were compared.【Results】There was no significant difference in the incidence of postoperative corneal scar between the two groups(P>0.05).There was no significant difference in the chart asymmetry index(P>0.05).The uncorrected visual acuity of the two groups of patients after surgery was significantly improved,and the corneal astigmatism and corneal topography chart asymmetry index were significantly reduced.The corneal topography asymmetry index was significantly better than that of the control group(P<0.05),and there was no significant difference in the VAS score between the two groups on the day after surgery,1d,5d and 10d after surgery(P>0.05).【Conclusion】Giant pterygium dry amniotic membrane coverage after excision combined with limbal stem cell transplantation can significantly relieve pain,improve uncorrected visual acuity and astigmatism,and promote corneal wound healing.It is recommended to popularize.
