摘 要:目的:分析并比较翼状胬肉切除术联合羊膜移植术与单纯翼状胬肉切除术对原发性翼状胬肉患者眼表泪膜稳定性的影响。方法:将确诊为原发性翼状胬肉的69例患者随机分为两组,其中观察组采用翼状胬肉切除术联合羊膜移植术治疗,对照组单纯采用翼状胬肉切除术治疗。对两组的泪膜破裂时间(break-up time,BUT)、泪液分泌试验(Schimer I test,SIt)及术后病情的复发率进行对比分析。结果:术后10 d两组患眼均拆除缝线,角膜、结膜创面均完全修复,术后均未见明显的角巩膜溃疡坏死及眼肌损伤等并发症发生。观察组术后1个月及3个月的BUT、SIt均高于对照组,差异有统计学意义(P<0.05)。观察组术后6个月病情的复发率为5.7%(2/35),显著低于对照组的26.5%(9/34),差异有统计学意义(P<0.05)。结论:翼状胬肉切除术联合羊膜移植术治疗原发性翼状胬肉较单纯翼状胬肉切除术疗效更确切,安全性更高,能快速缓解眼部刺激不适感,提高术后眼表泪膜的稳定性。Objective:To analyze and compare the effects of pterygium excision combined with amniotic membrane transplantation and simple pterygium excision on the stability of ocular surface tear film in patients with primary pterygium.Methods:69 patients diagnosed with primary pterygium were randomly divided into two groups.The observation group was treated with pterygium excision combined with amniotic membrane transplantation,and the control group was treated with pterygium excision alone.The tear film breakup time(BUT),Schimer I test(SIt)and postoperative recurrence rate of the two groups were compared and analyzed.Results:10 days after surgery,sutures were removed and corneal and conjunctival wounds were completely repaired.There were no obvious postoperative complications such as necrosis of cervicera ulcer and ocular muscle injury.The BUT and SIt of the observation group were higher than those of the control group 1 and 3 months after surgery,and the difference was statistically significant(P<0.05).The recurrence rate 6 months after operation in the observation group was 5.7%(2/35),significantly lower than 26.5%(9/34)in the control group,the difference was statistically significant(P<0.05).Conclusions:Pterygium excision combined with amniotic membrane transplantation is more effective and safer than simple pterygium excision in the treatment of primary pterygium.It can quickly relieve discomfort of eye irritation and improve the stability of ocular surface tear fil m after surgery.
