Kawase入路下显微镜与神经内镜观测性及可操作性的对比研究

摘　　要：目的研究显微镜与神经内镜在Kawase入路中的观测性及可操作性比较。方法选取2020年12月-2022年2月新疆医科大学第一附属医院神经外科中心的5例(10侧)尸头标本作为研究对象,模拟Kawase入路手术过程,通过显微镜和神经内镜观察相关解剖结构并对脑干腹侧区域观测距离和手术可操作距离进行测量,进而对比两种手术方式的差异性并作分析。结果与显微镜辅助Kawase入路比较,神经内镜辅助Kawase入路在脑干腹侧上方、前上方、前方、下方的观测距离和手术可操作距离均增大,差异有统计学意义(P<0.05),两组数据差异具有统计学意义;前下方的观测距离和手术可操作距离比较,差异无统计学意义(P>0.05)。结论神经内镜在Kawase入路中观察范围还是操作范围均优于显微镜,值得在临床工作中进一步推广。Objective To compare the observation and maneuverability of Kawase approach with using microscope and neuroendoscope.Methods 5 cadaveric head specimens(10 sides)from the Neurosurgery Center of the hospital from December 2020 to February 2022 were selected to simulate the operation process with Kawase approach.The relevant anatomical structures were observed by microscope and neuroendoscope,and the observation distance and operative distance of ventral brainstem were measured,and the differences between the two surgical methods were compared and analyzed.Results Compared with the microscope assisted Kawase approach,the observation distance and operative distance of the endoscope assisted Kawase approach in the above side,upfront,front and lower ventral part of the brainstem wereincreased,with a statistically significant difference(P<0.05);There was no significant difference between the observation distance at the lower front and the operative distance(P>0.05).Conclusion The scope of observation and operation of neuroendoscope in Kawase approach is better than that of microscope,so it is worth to further popularizing in clinical work.
