摘　　要：目的:探究与分析改良电休克(MECT)联合第二代抗精神病药治疗精神分裂症的效果及对脑电图、认知功能的影响。方法:采取随机数字表法对张家港市第四人民医院2020年2月-2022年2月收治的精神分裂症患者90例分组,每组45例。对照组给予奥氮平治疗,观察组在对照组基础上加用MECT治疗。对比两组疗效、阳性和阴性症状量表(PANSS)评分、精神分裂症认知功能成套测试共识版(MCCB)评分、淋巴细胞亚群、脑电图活动指标变化及不良反应发生情况。结果:观察组总有效率高于对照组(P<0.05)。两组治疗前PANSS评分、MCCB评分、淋巴细胞亚群及治疗前后脑电图活动指标相比,差异均无统计学意义(P>0.05);观察组治疗后MCCB各项评分、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均高于对照组,PANSS评分、CD8^(+)水平均低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率相比,差异均无统计学意义(P>0.05)。结论:MECT联合第二代抗精神病药治疗精神分裂症可有效促进改善临床症状及认知功能,机体免疫功能得以提升,安全性较高。Objective:To explore and analyze the efficacy of modified electroconvulsive(MECT)combined with second-generation antipsychotics in the treatment of schizophrenia and its influence on electroencephalogram(EEG)and cognitive function.Method:A total of 90 patients with schizophrenia who treated in Department of Psychiatry,the Fourth People’s Hospital of Zhangjiagang from February 2020 to February 2022 were divided into groups by random number table method,45 cases in each group.The control group was given the Anzapine treatment,the observation group was treated with MECT on the basis of the control group.The efficacy,positive and negative symptom scale(PANSS)score,measurement and treatment research to improve cognition in schizophrenia consensus cognitive battery(MCCB)score,lymphocyte subsets,changes of EEG activity indicators and incidence of adverse reactions were compared between two groups.Result:The total effective rate of the observation group was higher than that of control group(P<0.05).There were no significant differences in PANSS score,MCCB score,lymphocyte subsets and EEG activity indexes before and after treatment between two groups(P>0.05).After treatment,the MCCB scores,CD3^(+),CD4^(+),CD4^(+)/CD8^(+)levels in the observation group were higher than those in the control group,and the PANSS scores and CD8^(+)levels were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:MECT combined with second-generation antipsychotics in the treatment of schizophrenia can effectively improve clinical symptoms and cognitive function,improve the immune function of body,and it has high safety.
