简易手工旋切法在单人操作腹腔镜子宫肌瘤剥除术中的应用

摘　　要：目的:比较单人操作腹腔镜子宫肌瘤剔除术中简易手工旋切与电动旋切取出肌瘤的手术效果及术后恢复情况。方法:回顾性分析福建医科大学附属第二医院2019年3月-2021年6月实施单人操作腹腔镜子宫肌瘤剔除术患者258例,按手术方法不同将其分为简易手工旋切组(n=131)和电动旋切组(n=127),比较两组的取瘤时间、术中出血量、围手术期指标水平变化情况、术后恢复情况。结果:简易手工旋切组的取瘤时间为(9.71±3.83)min,明显短于电动旋切组的(12.61±3.45)min,差异有统计学意义(P<0.05)。简易手工旋切组的住院天数为(7.16±2.27)d,明显短于电动旋切组的(7.86±2.22)d,差异有统计学意义(P<0.05)。简易手工旋切组的住院费用为(15806.52±2467.75)元,明显少于电动旋切组的(17616.68±2778.82)元,差异有统计学意义(P<0.05)。结论:简易手工旋切法较电动旋切法具有取瘤时间更短、住院费用更少及住院时间更短的优势,可在腹腔镜子宫肌瘤剔除术中安全运用。Objective:To compare the surgical effect and postoperative recovery of simple manual rotary cutting and electric rotary cutting in the treatment of single-person operated laparoscopic myomectomy of uterus.Method:Retrospective analysis was made on 258 patients who underwent single-person operated laparoscopic myomectomy of uterus in the Second Affiliated Hospital of Fujian Medical University from March 2019 to June 2021.According to different surgical methods,they were divided into simple manual rotary cutting group(n=131)and electric rotary cutting group(n=127).The tumor removal time,intraoperative bleeding,changes in perioperative index levels,and postoperative recovery were compared between the two groups.Result:The tumor removal time in the simple manual rotary cutting group was(9.71±3.83)min,which was significantly shorter than that(12.61±3.45)min in the electric rotary cutting group,the difference was statistically significant(P<0.05).The length of hospital stay in the simple manual rotary cutting group was(7.16±2.27)d,which was significantly shorter than that(7.86±2.22)d in the electric rotary cutting group,the difference was statistically significant(P<0.05).The hospitalization cost in the simple manual rotary cutting group was(15806.52±2467.75)yuan,which was significantly lower than(17616.68±2778.82)yuan in the electric rotary cutting group,the difference was statistically significant(P<0.05).Conclusion:The simple manual rotary cutting has the advantages of shorter tumor removal time,less hospitalization cost and shorter hospitalization time than the electric rotary cutting method,and can be safely used in laparoscopic myomectomy of uterus.
