摘　　要：目的:观察Tei指数在α-地中海贫血(α-地贫)胎儿心功能评价中的应用。方法:选取2019年1月-2021年11月于广东医科大学顺德妇女儿童医院产前诊断中心行地贫基因诊断证实胎儿α-地中海贫血单胎孕妇80例为研究对象,作为观察组,并选取同期于本院进行产检的正常单胎孕妇50例作为对照组。采用超声心动图测量胎儿左、右心室的Tei指数,并利用受试者工作特征曲线(ROC)分析Tei指数对α-地贫胎儿心功能的评估价值。结果:80例α-地贫胎儿经羊水穿刺术行地贫基因检测,静止型α-地贫15例,中间型α-地贫12例,重型α-地贫20例,轻型α-地贫33例。重型α-地贫患者左心室、右心室Tei指数高于中间型、轻型、静止型α-地贫及对照组,中间型α-地贫患者左心室、右心室Tei指数高于轻型、静止型α-地贫及对照组(P<0.05),轻型、静止型α-地贫及对照组左心室、右心室Tei指数比较差异无统计学意义(P>0.05)。ROC曲线分析显示,左心室Tei指数评估α-地贫胎儿心功能的敏感度为96.25%,特异度为100%,AUC为0.988;右心室Tei指数评估α-地贫胎儿心功能的敏感度为93.75%,特异度为98.00%,AUC为0.978。结论:对α-地贫胎儿进行Tei指数检测,对胎儿心功能评估具有较高诊断价值。Objective:To observe the application of Tei index in the evaluation of fetal cardiac function withα-thalassemia.Method:A total of 80 singleton pregnant women with fetalα-thalassemia confirmed by thalassemia gene diagnosis in the Prenatal Diagnosis Center of Shunde Women’s and Children’s Hospital,Guangdong Medical University from January 2019 to November 2021 were selected as the research object and served as the observation group,and 50 normal singleton pregnant women who underwent prenatal examination in our hospital during the same period were selected as the control group.The Tei index of fetal left and right ventricles was measured by echocardiography,and the value of Tei index in evaluating fetal cardiac function withα-thalassemia was analyzed by receiver operating characteristic curve(ROC).Result:A total of 80 fetuses withα-thalassemia underwent amniocentesis for thalassemia gene detection,there were 15 cases of quiescentα-thalassemia,12 cases of intermediateα-thalassemia,20 cases of severeα-thalassemia and 33 cases of mildα-thalassemia.The left ventricular and right ventricular Tei index of patients with severeα-thalassemia was higher than that of intermediate type,mild type,quirer typeα-thalassemia and control group,and the left ventricular and right ventricular Tei index of patients with intermediate typeα-thalassemia was higher than that of mild type,quirer typeα-thalassemia and control group(P<0.05).There was no significant difference in the Tei index of left ventricle and right ventricle between mild and quiescentα-thalassemia and control group(P>0.05).ROC curve analysis showed that the sensitivity,specificity and AUC of left ventricular Tei index in evaluating fetal cardiac function withα-thalassemia were 96.25%,100%and 0.988,respectively.The sensitivity,specificity and AUC of right ventricular Tei index in evaluating fetal cardiac function withα-thalassemia were 93.75%,98.00%and 0.978,respectively.Conclusion:The detection of Tei index inα-thalassemia fetuses has high diagnostic v
