腹腔镜模拟训练教学在中医住院医师规范化培训的应用

摘　　要：目的探讨在中医住院医师规范化培训阶段,应用腹腔镜模拟训练教学法的效果。方法选择安徽中医药大学第一附属医院规培基地2021年1月—2021年3月的中医住院规培医师40人,培训前均未学习过腹腔镜技术,对比采用腹腔镜模拟训练教学方法前后,中医住院规培医师掌握的理论知识及腹腔镜训练的操作情况。腹腔镜操作训练包括传递操作(夹豆训练)、剪切操作(剪同心圆训练)、缝合打结操作,连续培训4周后,对每位住院医师的腹腔镜操作计时、计数,测试培训后上述各项技能的完成情况,比较操作时间。结果参加腹腔镜模拟训练教学培训后,中医住院规培医师的腹腔镜基础理论分值明显高于培训前;腹腔镜模拟操作完成时间明显缩短,其中传递操作、剪切操作、缝合打结操作水平显著提升,差异均有统计学意义(P<0.05)。结论在中医住院医师规范化培训中,应用腹腔镜模拟训练教学可提高中医住院规培医师的理论水平、操作技能,腹腔镜基本操作技能会得到持续显著的提升,完成操作时间明显缩短。Objective To observe effect of the level of laparoscopic surgery which by applying methods of laparoscopic simulation training teaching in the stage of standardized training for residents of traditional Chinese medicine.Methods A total of 40 doctors were selected from the training base of the First Affiliated Hospital of Anhui University of Chinese Medicine from January 2021 to March 2021.They had no experience in laparoscopic training.The effect of the level of laparoscopic surgery,which by applying methods of laparoscopic simulation training teaching,Laparoscopic operation training includes transfer operation(bean clamping training),shear operation(shear concentric circle training),suture and knot tying operation was observed.The operating time of each physician was compared after 4 weeks.Results After taking part in laparoscopic simulation teaching training,the score of laparoscopic basic theory of TCM inpatient training was significantly higher than that before training;The completion time of laparoscopic simulation operation was significantly shortened,and the levels of transfer operation,shear operation,suture and knot operation were significantly improved,with statistical significance(P<0.05).Conclusion The application of laparoscopic simulation teaching can improve their theoretical level and operational skills in the standardized training of residents of traditional Chinese medicine.Laparoscopic basic operation skills will be significantly improved and the completion time will be significantly shortened with the extension of training time.
