摘　　要：慢性萎缩性胃炎(chronic atrophic gastritis,CAG)是一种病势反复、缠绵难愈的消化系统疾病,是常见的胃癌前病变状态,其特征症状不明显,容易与胃炎混淆以致误诊、漏诊,导致患者转化为胃癌的风险上调,故CAG应当及早进行干预。然而,该病暂未出现理想的特效化学药物,西医临床以对症治疗为主。中医药根据临床辨证分型对CAG实行随证治疗,疗效确切,安全性良好。值得注意的是,CAG属于中医学“胃痛”“痞满”“嘈杂”等范畴,其核心病机在于本虚标实。患者脾胃虚弱,可受到湿邪、热邪、寒邪等侵袭而出现CAG症状,临床辨证不一,多见不同证型夹杂。由此可知,CAG的中医治疗不应拘泥于单种治则,需根据患者具体证型辨证施治,并在针对各种实证的前提下,同时补益脾胃正气,以防在驱邪的过程中脾胃之本进一步受损,导致运化功能愈发恶化。该文基于中医理论对CAG不同证型的诠释,对近年中医药治疗CAG的报道兹以综述,以期为疾病在临床实践中更好地应用中医药提供参考。Chronic atrophic gastritis(CAG)is a recurrent,lingering and refractory digestive system disease. It is a common precancerous state of gastric cancer. Its characteristic symptoms are not obvious,which is easy to be confused with gastritis,resulting in misdiagnosis and missed diagnosis,leading to the up regulation of the risk of patients turning into gastric cancer. Therefore,CAG should be intervened as soon as possible. However,there is no ideal specific chemical drug for this disease,and symptomatic treatment is the main clinical practice of Western medicine. Traditional Chinese medicine carries out syndrome following treatment on CAG according to clinical syndrome differentiation,which has definite curative effect and good safety. It is worth noting that CAG belongs to the categories of“stomach pain”“fullness”and“noise”in traditional Chinese medicine,and its core pathogenesis lies in the deficiency and excess.Patients with weak spleen and stomach can be invaded by dampness evil,heat evil and cold evil to show CAG symptoms. Clinical syndrome differentiation is different,and different syndrome types are often mixed.It can be seen that the traditional Chinese medicine treatment of CAG should not stick to a single treatment principle,but should be treated according to the specific syndrome types of patients,and on the premise of various empirical evidence,at the same time,replenish the spleen and stomach healthy Qi,so as to prevent the further damage of the essence of the spleen and stomach in the process of expelling evil,resulting in the deterioration of transportation function. Based on the interpretation of different syndrome types of CAG by traditional Chinese medicine theory,this paper summarizes the reports on the treatment of CAG by traditional Chinese medicine in recent years,in order to provide reference for the better application of traditional Chinese medicine in clinical practice.
