胰腺癌组织CTTN和miR-545-3p表达水平及其与临床病理和预后的相关性研究

摘　　要：目的探究胰腺癌组织中皮层肌动蛋白(cortical actin,CTTN)、微小核糖核酸(micro RNA,miR)-545-3p的表达情况及其与患者预后的相关性。方法募集2016年1月~2018年12月南通大学附属医院行腹腔镜胰腺癌切除手术的胰腺癌患者138例,采集胰腺癌组织和癌旁正常组织,通过实时荧光定量PCR(real-time quantitative PCR,qRT-PCR)检测CTTN mRNA和miR-545-3p表达,二者的相关性通过Pearson法进行分析,分析CTTN mRNA和miR-545-3p表达与临床病理特征的关系,绘制Kapplan-Meier生存曲线分析胰腺癌组织中CTTN mRNA和miR-545-3p表达与患者生存率的关系,经COX回归对影响胰腺癌患者生存的危险因素进行分析。结果与癌旁正常组织比较,胰腺癌组织中CTTN mRNA表达上调(2.11±0.87 vs 1.02±0.46),miR-545-3p表达下调(0.45±0.19 vs 0.97±0.42),差异均有统计学意义(t=12.895,13.251,均P=0.000);胰腺癌组织中CTTN mRNA和miR-545-3p之间具有负相关性(r=-0.410,P<0.05);CTTN mRNA和miR-545-3p表达均与肿瘤分化程度、TNM分期以及淋巴结转移有关(t=2.473~11.418,均P<0.05)。根据CTTN mRNA和miR-545-3p表达量的均值将所有患者依次分为CTTN mRNA高表达组和CTTN mRNA低表达组,miR-545-3p高表达组和miR-545-3p低表达组。生存曲线分析得出,CTTN mRNA低表达组三年生存率高于CTTN mRNA高表达组(24.62%vs 9.59%),miR-545-3p高表达组三年生存率高于miR-545-3p低表达组(25.37%vs 8.45%),差异均有统计学意义(χ^(2)=4.560,P=0.033;χ^(2)=5.941,P=0.015)。存在淋巴结转移、高CTTN mRNA表达、低miR-545-3p表达是胰腺癌患者术后三年死亡的危险因素(Waldχ^(2)=1.885,1.562,0.625,均P<0.05)。结论胰腺癌组织中CTTN mRNA高表达和miR-545-3p低表达均与患者预后不良有关。Objective To investigate the expressions of cortical actin(CTTN)and micro RNA(miR)545-3p in pancreatic cancer tissues and their correlation with the prognosis of patients.Methods A total of 138 pancreatic cancer patients who underwent laparoscopic pancreatic cancer resection in Affiliated Hospital of Nantong University from January 2016 to December 2018 were recruited,and their pancreatic cancer tissues and adjacent normal tissues were collected.The expressions of CTTN mRNA and miR-545-3p were measured by qRT-PCR,and the correlation between the two was analyzed by Pearson method.The relationship between CTTN mRNA,miR-545-3p expression and clinicopathological features was analyzed,Kapplan-Meier survival curve was drawn to analyze the relationship between CTTN mRNA and miR-545-3p expressions in pancreatic cancer tissue and the survival rate of patients,and the risk factors affecting the survival of pancreatic cancer patients were analyzed by COX regression.Results Compared with adjacent normal tissues,the expression of CTTN mRNA in pancreatic cancer tissues was up-regulated(2.11±0.87 vs 1.02±0.46),and the expression of miR-545-3p was down-regulated(0.45±0.19 vs 0.97±0.42),the differences were statistically significant(t=12.895,13.251,all P=0.000).There was a negative correlation between CTTN mRNA and miR-545-3p in pancreatic cancer tissue(r=-0.410,P<0.05).The expressions of CTTN mRNA and miR-545-3p were related to tumor differentiation,TNM stage and lymph node metastasis(t=2.473~11.418,all P<0.05).According to the mean of CTTN mRNA and miR-545-3p expressions,all patients were divided into CTTN mRNA high expression group and CTTN mRNA low expression group,miR-545-3p high expression group and miR-545-3p low expression group.Survival curve analysis showed that the 3-year survival rate of CTTN mRNA low expression group was higher than that of CTTN mRNA high expression group(24.62%vs 9.59%),and the 3-year survival rate of miR-545-3p high expression group was higher than that of miR-545-3p low expression group(25.37%
