RGPCL治疗青少年近视合并高度散光临床效果及对角膜组织形态的影响

摘　　要：目的 探讨硬性透气性角膜接触镜治疗青少年近视合并高度散光的临床效果及对角膜组织形态的影响。方法 将80例青少年近视合并高度散光患者中佩戴硬性透气性角膜接触镜的42例患者设为研究组,佩戴框架眼镜的38例患者设为对照组。比较治疗前后两组患者视力、屈光度、眼轴长度、中央角膜各层厚度和角膜地形图参数,并评估其安全性。结果 治疗后研究组患者视力显著优于对照组(P<0.01),屈光度和眼轴长度显著优于对照组(P<0.05或0.01)。治疗后研究组患者仅中央角膜上皮层厚度较治疗前显著变薄(P<0.05),对照组患者中央角膜各层厚度较治疗前均无显著变化(P>0.05),治疗前后两组组间比较差异无统计学意义(P>0.05)。治疗后两组患者角膜曲率、Steep K和Flat K较治疗前无显著变化(P>0.05),组间比较差异无统计学意义(P>0.05)。对照组佩戴框架眼镜期间无明显不良反应;研究组患者治疗期间出现过敏性结膜炎3例(6眼),角膜上皮点染2例(3眼),角膜擦伤1例(1眼),经对症处理后均恢复。结论 硬性透气性角膜接触镜治疗青少年近视合并高度散光可提高患者裸眼视力,降低屈光度,对角膜形态无明显影响,短期安全性较高。Objective To study the clinical effect of rigid gas permeable contact lens(RGPCL) on correcting adolescents with myopia and high astigmatism and its influences on corneal tissue morphology. Methods A total of 80 adolescent patients with myopia and high astigmatism were selected, among which 42 patients wearing RGCPL were set as the study group and 38 patients wearing frame glasses were set as the control group. The changes in visual acuity, diopter, axial length, thickness of central corneal layers and corneal topography parameters before and after treatment were compared between the two groups. And safety in both groups was evaluated. Results After treatment, the visual acuity of the study group was significantly better than that of the control group(P<0.01), and the diopter and axial length were significantly better than those of the control group(P<0.05 or 0.01). After treatment, the thickness of central corneal epithelium layer of the study group was significantly thinner than that before treatment(P<0.05). The central corneal thickness of the control group had no significant changes compared with that before treatment(P>0.05), and there was no significant difference between the two groups before and after treatment(P>0.05). There were no significant changes in corneal curvature, steep keratometry(Steep K) and flat keratometry(Flat K) between the two groups after treatment(P>0.05), and there was no significant difference between the two groups(P>0.05). The control group had no obvious adverse reaction during wearing frame glasses. In the study group, 3 cases of allergic conjunctivitis(6 eyes), 2 cases of corneal epithelial infection(3 eyes), and 1 case of corneal abrasion(1 eye) occurred during treatment,all of them recovered after symptomatic treatment.Conclusions The application of RCGPL in adolescents with myopia and high astigmatism can better improve uncorrected visual acuity,and reduce diopter,without significant effects on corneal morphology and with high short-term safety.
