屈光度数>-6.00D的高度近视患者应用经上皮准分子激光屈光性角膜切削术矫治的临床效果观察

摘　　要：目的 研究屈光度数>-6.00D的高度近视患者应用经上皮准分子激光屈光性角膜切削术(TPRK)矫治的临床效果。方法 选取181例高度近视患者的屈光度数大于-6.00D共241眼为本研究对象,按照随机数表法分为对照组90例患者(118眼)和实验组91例患者(123眼),对照组患眼使用飞秒激光制瓣辅助下的准分子激光原位角膜磨镶术(FS-LASIK)治疗。实验组患眼使用TPRK进行治疗,比较两组患眼术后上皮愈合时间。比较术后3个月后患眼视力[最佳矫正视力(BCVA)、角膜后表面高度、屈光度]变化。比较术后14d内两组患眼手术并发症[感染、角膜穿透、角膜上皮下雾状浑浊]发生率。结果 术后实验组患眼上皮愈合时间明显长于对照组患者(P<0.05)。术后3个月后,两组患眼BCVA、角膜后表面高度较治疗前均有明显升高,两组患眼BCVA、角膜后表面高度比较差异无统计学意义(P<0.05);两组患者屈光度较治疗前均有明显降低,两组患眼屈光度比较差异无统计学意义(P<0.05)。术后14d内两组患者均未发生术后并发症。结论 使用TPRK治疗屈光度数>-6.00D的高度近视患者术后患眼上皮愈合较慢,视力恢复效果优良,且具有较高安全性。Objective To study the clinical effect of epithelial excimer laser photorefractive keratectomy(TPRK)on high myopia patients with refractive power >-6.00 d.Methods 241 eyes of 181 patients with high myopia with refractive power greater than-6.00D were selected as the research objects.90 patients(118 eyes)in the control group and 91 patients(123 eyes)in the experimental group were randomly divided.Patients in the control group were treated with excimer laser in situ keratomileusis(FS-LASIK)assisted by femtosecond laser flap making.The experimental group was treated with TPRK,and the epithelial healing time was compared between the two groups.The changes of visual acuity(BCVA,corneal posterior surface height,diopter)were compared 3 months after operation.The incidence of surgical complications(infection,corneal penetration,corneal haze under the upper and lower skin)in the two groups were compared within 14 days after operation.Results After operation,the healing time of the epithelium in the experimental group was significantly longer than that in the control group(P<0.05).Three months after operation,The BCVA and corneal posterior surface height of the two groups were significantly higher than those before treatment,but there was no significant differ ence between the two groups(P<0.05).Compared with before treatment,the diopter of patients in both groups decreased significantly,but there was no significant difference between the two groups(P<0.05).No postoperative complications occurred in the two groups within 14 days after operation.Conclusion The refractive power of TPRK is>-6.00 days after operation,the epithelial healing of the affected eye is slow,the visual recovery effect is excellent,and it has high safety.
