摘　　要：目的:探讨龈沟液网膜素-1(Omentin-1)、趋化因子CX3C的配体1(chemokine CX3C ligand 1,CX3CL1)、巨噬细胞集落刺激因子受体(macrophage colony stimulate factor receptor,M-CSFR)在慢性牙周炎患者中的水平及与病情活动性的相关性。方法:选取2018年2月~2021年2月我院慢性牙周炎患者58例作为观察组,选取同期健康体检者58例作为对照组。比较两组一般资料及龈沟液Omentin-1、CX3CL1、M-CSFR水平,分析慢性牙周炎影响因素及慢性牙周炎患者龈沟液Omentin-1、CX3CL1、M-CSFR水平与牙龈指数(GI)、附着丧失程度(AL)、牙周袋深度(PD)相关性,Logistic回归分析慢性牙周炎患者病情活动性的影响因素,采用受试者工作特征(ROC)曲线分析龈沟液Omentin-1、CX3CL1、M-CSFR评估慢性牙周炎患者病情活动性的价值。结果:两组年龄、刷牙次数、吸烟史、GI、AL、PD及龈沟液Omentin-1、CX3CL1、M-CSFR水平比较,差异有统计学意义(P<0.05)。Logistic回归分析显示,校正年龄、刷牙次数、吸烟史后,CX3CL1、M-CSFR升高及Omentin-1降低均为慢性牙周炎危险因素(P<0.05)。龈沟液Omentin-1水平与GI、AL、PD呈负相关,CX3CL1、M-CSFR水平与GI、AL、PD呈正相关(P<0.05)。校正吸烟史、GI、AL、PD后,CX3CL1、M-CSFR升高及Omentin-1降低均为慢性牙周炎患者病情处于活动期的危险因素(P<0.05)。ROC曲线分析显示,龈沟液CX3CL1、M-CSFR、Omentin-1联合评估慢性牙周炎病情活动性的AUC值为0.891,高于单独预测。结论:慢性牙周炎患者龈沟液CX3CL1、M-CSFR、Omentin-1均异常表达,且与牙周组织破坏程度及病情活动性有关,共同参与慢性牙周炎发病过程。Objective:To explore the Omentin-1,chemokine CX3C ligand 1(CX3CL1)and macrophage colony stimulating factor receptor(M-CSFR)levels of gingival crevicular fluid in chronic periodontitis patients and correlation with disease activity.Methods:From February 2018 to February 2021,58 patients with chronic periodontitis in our hospital were selected as the observation group,and 58 patients with healthy physical examination during the same period were selected as the control group.The general information and the levels of gingival crevicular fluid Omentin-1,CX3CL1 and M-CSFR were compared between the two groups.The influencing factors of chronic periodontitis and the correlation between the levels of gingival crevicular fluid Omentin-1,CX3CL1 and M-CSFR in patients with chronic periodontitis,gingival index(GI),degree of attachment loss(AL) and periodontal pocket depth(PD) were analyzed.Logistic regression was used to analyze the influencing factors of disease activity in patients with chronic periodontitis.The receiver operating characteristic(ROC) curve was used to analyze the value of gingival crevicular fluid Omentin-1,CX3CL1 and M-CSFR in assessing the activity of patients with chronic periodontitis.Results:There was statistically significant difference between the two groups of age,number of tooth brushing,smoking history,GI,AL,PD,and levels of Omentin-1,CX3CL1 and M-CSFR(P<0.05).After controlling for age,tooth brushing frequency and smoking history,the CX3CL1,M-CSFR increase and Omentin-1 decrease are risk factors for patients with chronic periodontitis in the active stage(P<0.05).The level of Omentin-1 in gingival crevicular fluid was negatively correlated with GI,AL,PD.CX3CL1 and M-CSFR was positively correlated with GI,AL,PD(P<0.05).After controlling smoking history,GI,AL and PD,the CX3CL1,M-CSFR,Omentin-1 were still related to the disease activity of chronic periodontitis patients(P<0.05).The AUC value of gingival crevicular fluid CX3CL1,M-CSFR,Omentin-1 combined to assess the activity of chronic periodontitis w
