四种机用镍钛器械预备磨牙根管的临床效果分析

摘　　要：目的:比较镍钛器械M3-L Platinum、HyFlex EDM、WaveOne Gold和Reciproc Blue磨牙根管预备的临床效果。方法:选择牙髓炎或根尖周炎需要根管治疗的磨牙144颗,随机分为4组,M组使用M3-L顺时旋转预备根管;H组使用HyFlex EDM顺时旋转预备根管;W组使用WaveOne Gold往复运动预备根管;R组使用Reciproc Blue往复运动预备根管。均使用热牙胶垂直加压技术充填根管。比较4种镍钛器械根管预备时间,术后疼痛,根管预备及根充效果,器械折断情况。结果:M组预备时间最短,R组预备时间最长,差异有统计学意义(P<0.05);4组在预备后均能形成锥度及流畅度好的根管形态,术后疼痛及根充效果均无统计学意义(P>0.05)。M组使用中出现变形,H组、W组、R组均有器械折断。结论:M3-L、HyFlex EDM、WaveOne Gold和Reciproc Blue在预备磨牙根管时,均能使根管获得良好的预备形,出现较轻的术后疼痛,使用时都需注意器械折断的发生,M3-L预备磨牙根管更加省时。Objective:To compare the preparation effects by using four types of nickel-titanium instruments(M3-L Platinum/HyFlex EDM/WaveOne Gold/Reciproc Blue)in preparation of molar root canals.Methods:Totally 144 molars with pulpitis or periapical periodontitis were randomly divided into 4 groups.Group M was treated with M3-L and group H with HyFlex EDM by rotary motion.Group W was treated with WaveOne Gold and group R was prepared with Reciproc Blue by reciprocating motion.All the root canals were filled by vertical compression technique.Root Canal preparation time,postoperative pain,root canal preparation,root canal filling effect and instrument fracture were compared.Results:The preparation time in group M was the shortest and the longest in group R,and the difference was statistically significant(P<0.05).In 4 groups,the root canal shape with good taper and smoothness was formed after preparation.There was no statistically significant difference among 4 groups in postoperative pain and root canal filling effect.Conclusion:M3-L,HyFlex EDM,WaveOne Gold and Reciproc Blue can make the root canal get a good shape when preparing the molar root canal,and have less pain after operation.When using,we should pay attention to the instrument fracture.M3-L preparation of molar root canal is more time-saving.
