摘  要:目的:探讨超声弹性成像(ultrasound elastography,UE)联合甲状腺影像学报告及数据系统(thyroid imaging reporting and data system,TI-RADS)在鉴别诊断良恶性甲状腺结节中的应用价值。方法:选取我院2020年5月至2021年2月期间经手术病理证实的102例甲状腺结节患者(136个结节)作为研究对象,所有研究对象术前均接受UE分级评分、TI-RADS分级标准诊断及二者的联合评分诊断,构建受试者工作特征曲线,计算曲线下面积(area under the ROC curve,AUC),分析诊断效能。结果:UE诊断甲状腺恶性结节的灵敏度、特异度、准确度分别为:86.67%、86.81%、86.76%,TI-RADS分类诊断甲状腺恶性结节的灵敏度、特异度、准确度分别为71.11%、84.62%、80.15%,而二者联合诊断甲状腺恶性结节的灵敏度、特异度、准确度分别为:91.11%、89.01%、89.71%,二者联合的诊断效能最高。UE、TI-RADS分类及二者联合诊断甲状腺恶性结节的AUC分别为:0.918、0.841、0.950,二者联合诊断明显高于其单独应用,差异均有统计学意义(P<0.05)。结论:UE联合二维超声TI-RADS分级标准对良恶性甲状腺结节的鉴别诊断具有较高的诊断效能,两者的联合应用可弥补单一技术在鉴别诊断中的缺陷与不足,可以显著提高诊断效能。Objective:To analyze the application of ultrasonic elastography(UE)combined with thyroid imaging reporting and data system(TI-RADS)in the differential diagnosis of benign and malignant thyroid nodules.Methods:102 patients with thyroid nodules(136 nodules)confirmed by pathology in our hospital from May 2020 to February 2021 were selected as the research objects.All the research objects were diagnosed by the solo and combined use of UE and TI-RADS.Receiver operating characteristic(ROC)curve was constructed,and the area under the curve(AUC)was calculated to analyze the diagnostic efficacy.Results:The sensitivity,specificity and accuracy of UE in the diagnosis of thyroid malignant nodules were 86.67%,86.81%and 86.76%,respectively;those of TI-RADS were 71.11%,84.62%and 80.15%,respectively;those of UE together with TI-RADS were 91.11%,89.01%and 89.71%,respectively,which showed the highest diagnostic efficiency.AUC of UE,TI-RADS and the combination group were 0.918,0.841 and 0.950,respectively.The combined use of UE and TI-RADS was significantly more effective than the solo use of them(P<0.05).Conclusion:Ultrasonic elastography combined with TI-RADS is highly effective in the differential diagnosis of benign and malignant thyroid nodules.The combined application of the two can make up for the defects and shortcomings of a single technology in the differential diagnosis,and can significantly improve the diagnostic efficiency.
