颅缝早闭婴幼儿手术治疗前后发育筛查结果分析

摘　　要：目的探讨颅缝早闭患儿在手术治疗前后发育筛查结果的变化。方法回顾研究2017年2月至2019年2月在复旦大学附属儿科医院整形外科收治的36例颅缝早闭患儿的病例资料。采用儿童发育筛查测验(development screening test,DST)量表,分别在患儿术前1d、术后3个月进行测试并记录结果。评估患儿手术前后运动、社会适应、智力变化,探究颅缝早闭患儿智力的影响因素。结果患儿术后总发育商得分为(60.2±5.76)分,智商得分为(63.9±7.01)分,均高于术前(52.4±10.92)分和(51.7±10.61)分。具体表现为运动(10.62±2.71)分、社会适应(12.44±1.38)分、智力(29.57±2.49)分,明显高于术前(6.71±1.23)分、(8.62±2.05)分、(17.09±2.58)分,术前术后相比差异具有统计学意义(P<0.05)。结论颅缝早闭术不仅可以扩大颅腔,改善外观,还对患儿的运动能力、社会适应能力以及智力的健康发展起到重要的作用。Objective To explore the changes of developmental screening results in children with craniosynostosis before and after operations.Methods From February 2017 to February 2019,retrospective review was conducted for 36 children hospitalized with craniosynostosis.Development screening test(DST)scale was utilized for evaluating the changes of movement,social adaptation and intelligence before and after operations and exploring the influencing factors of intelligence.Results The scores of total development quotient(DQ)and intelligence quotient were(60.2±5.76),(63.9±7.01)at pre-operation and(52.4±10.92),(51.7±10.61)at post-operation.And exercise(10.62±2.71),social adaptation(12.44±1.38)and intelligence(29.57±2.49)at pre-operation were significantly higher than those(6.71±1.23),(8.62±2.05),(17.09±2.58)at post-operation.And the differences were statistically significant(P<0.05).Body mass index(BMI),operative mode,operative duration and anesthetic duration were correlated with the level of development recovery.Conclusions Craniotomy can not only expand cranial cavity and improve the appearance,but also play an important role in sport ability,social adaptability and healthy development of intelligence of children.
