摘 要:目的:探讨^(68)Ga-DOTATATE PET/CT显像联合人嗜铬蛋白A (CgA)对神经内分泌肿瘤(NET)患者的疗效监测作用。方法:2018年6月—2019年6月,选取西南医科大学附属医院经病理确诊的80例NET患者,均进行^(68)Ga-DOTATATE PET/CT检测并采用免疫放射法检测血清中CgA水平,观察患者疾病诊断及预后、疗效分析。结果:NET患者的血清CgA水平平均值为222.35ng/mL,T1期患者的CgA水平低于T2患者(P<0.05),与T2分期相比,T3分期患者血清CgA水平升高(P<0.05),与T3分期相比,T4分期患者血清CgA水平升高(P<0.05)。NET患者中26例患者合并淋巴结转移,54例未淋巴结转移,合并淋巴结转移患者中血清中的CgA水平高于未淋巴结转移患者(P<0.05);CgA阳性患者48例,随访2年有22例(45.83%)存活,CgA阴性患者32例,随访2年有24例(75.00%)存活,CgA阳性患者存活率低于阴性患者(P=0.009);2年期间CgA阳性患者中14例(29.16%)复发,CgA阴性患者有2例(6.25%)复发,CgA阳性及阴性复发率比较存在显著差异(P=0.012);NET患者T/Lratio水平与CgA水平呈正相关性(r=0.412,P<0.05)。结论:^(68)Ga-DOTATATE PET/CT显像能够提高NET诊断效果,CgA水平会随着疾病病理分期及淋巴结转移而升高,两者均对NET患者分期及预后有较好的诊断效果。Purpose:To explore the efficacy monitoring of ^(68)Ga-DOTATATE PET/CT imaging combined with human chromoprotein A(CgA)in neuroendocrine tumors.Methods:From June 2018 to June 2019,80 pathologically confirmed patients with neuroendocrine tumor in affiliated hospital of southwest medical university were selected.All patients underwent^(68)Ga-DOTATATE PET/CT,serum CgA level was detected by radioimmunology method,the disease diagnosis,prognosis and therapy efficacy were analyzed.Results:The mean level of serum CgA in NET patients was 222.35 ng/mL,the level of CgA in T1 patients was lower than that in T2 patients(P<0.05),the level of serum CgA in T3 patients was higher than that in T2 patients(P<0.05),and the level of serum CgA in T4 patients was higher than that in T3 patients(P<0.05).Of all the NET patients,26 were with lymph node metastasis,54patients without lymph node metastasis,the positive rate of serum CgA in patients with lymph node metastasis was higher than that in patients without lymph node metastasis(P<0.05).Of the 48 CgA-positive patients,22(45.83%)were alive in the 2-year follow-up,of the 32 CgA-negative patients,24(75.00%)were alive in the 2-year follow-up.The survival rate of CgA-positive patients was lower than that of negative patients(P=0.009);During 2 years,14(29.16%)cases of CgA-positive patients relapsed,and 2(6.25%)cases of CgA-negative patients had recurrences.There was a significant difference between CgA-positive and CgA-negative patients in recurrence rates(P=0.012);T/L ratio level was positively correlated with CgA level in NET patients(r=0.412,P<0.05).Conclusion:^(68)GaDOTATATE PET/CT imaging can be used to improve the diagnostic effect of neuroendocrine tumor,higher CgA level is related to the high pathological stage of the disease and lymph node metastasis,both of which have good diagnostic effect on the stage and prognosis of patients with neuroendocrine tumor.
