何氏胃疡方联合质子泵抑制剂治疗胃溃疡的效果及对炎性因子、胃黏膜屏障功能的影响

摘　　要：目的探讨何氏胃疡方联合质子泵抑制剂(PPI)治疗胃溃疡的效果及对炎性因子、胃黏膜屏障功能的影响。方法选取2018年12月—2021年12月收治的102例胃溃疡为研究对象,按照治疗方案不同分为研究组和对照组,每组51例。对照组给予PPI治疗,研究组在对照组的基础上给予何氏胃疡方治疗。观察2组治疗效果,比较2组治疗前后中医证候积分、血清炎性因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]、胃黏膜屏障功能指标[黏膜厚度评分、腺体密度评分、血管内皮生长因子(VEGF)、转化生长因子-β(TGF-β)]、血清胃肠激素[胃泌素(GAS)、胃动素(MTL)、胆囊收缩素(CCK)]水平及不良反应。结果研究组治疗总有效率高于对照组(P<0.05)。治疗4周后,2组血清CRP、IL-6、TNF-α、GAS、MTL、CCK水平及黏膜厚度评分、腺体密度评分、中医证候积分均较治疗前降低,研究组低于对照组;2组胃黏膜VEGF、TGF-β表达水平较治疗前升高,研究组高于对照组(P<0.05)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论何氏胃疡方联合PPI治疗胃溃疡能明显提高疗效,缓解临床症状,减轻炎症反应,改善胃黏膜屏障功能,调节胃肠激素,且不增加药物不良反应。Objective To investigate the effect of He's Weiyang Formula combined with proton pump inhibitor(PPI)on gastric ulcer and its impact on inflammatory factors and gastric mucosal barrier function.Methods In total,102 patients with gastric ulcer treated from December 2018 to December 2021 were selected as the research subjects,and were divided into the research group(n=51)and the control group(n=51)according to different treatment schemes.The control group was treated with PPI,and the research group was treated with He's Weiyang Formula on the basis of the control group.The therapeutic effect of the two groups was observed,and the TCM syndrome score,serum inflammatory factors[C reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)],gastric mucosal barrier function indexes[mucosal thickness score,gland density score,vascular endothelial growth factor(VEGF),transforming growth factor-β(TGF-β)],serum gastrointestinal hormones[gastrin(GAS),motilin(MTL),cholecystokinin(CCK)]levels before and after treatment and adverse reactions were compared between the two group.Results The total effective rate of the research group was higher than that of the control group(P<0.05).At 4 weeks after treatment,the serum levels of CRP,IL-6,TNF-α,GAS,MTL,CCK,and mucosal thickness score,gland density score,TCM syndrome score of the two groups were lower than those before treatment,and lower in the research group than in the control group;the expression levels of VEGF and TGF-βin gastric mucosa were significantly higher in both groups than those before treatment,and higher in the research group than in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion He's Weiyang Formula combined with PPI can significantly improve the therapeutic effect of gastric ulcer,alleviate clinical symptoms,reduce inflammatory reaction,improve gastric mucosal barrier function,regulate gastrointestinal hormones,and do not increase adverse drug
