摘  要:目的 分析幽门螺杆菌(Hp)感染与胃镜下慢性胃炎病理类型关系及其对肠道菌群、胃黏膜组织中微小RNA-22(miR-22)和NOD样受体蛋白3(NLRP3)炎症小体表达的影响。方法 选取2018年7月-2019年6月中国医科大学附属第四医院诊治的慢性胃炎患者84例,根据是否感染Hp分为Hp感染组(n=27)和未感染组(n=57)。比较不同慢性胃炎类型及病理变化Hp阳性率。采用对数值计算患者粪便中菌群数量。免疫放射比浊检测血清C-反应蛋白(CRP)及血清淀粉样蛋白A(SAA)。电化学发光法检测血清降钙素原(PCT)。酶联免疫吸附法检测血清白细胞介素-10(IL-10)和肿瘤坏死因子-α(TNF-α)。Western Bolt检测胃黏膜miR-22、NLRP3炎症小体、凋亡相关斑点蛋白(ASC)和半胱氨酸天冬氨酸蛋白酶1(Caspase-1)水平。结果 不同病理类型、炎性程度、炎性活动度以及固有层淋巴滤泡的Hp阳性率差异有统计学意义(P<0.05);感染组肠道双歧杆菌、大肠埃希菌和乳酸杆菌数量低于非感染组,嗜酸乳杆菌高于非感染组(P<0.05);感染组CRP、PCT、SAA、TNF-α高于非感染组,IL-10低于非感染组(P<0.05);感染组miR-22低于非感染组,NLRP3、ASC和Caspase-1高于非感染组(P<0.05)。结论 Hp感染可能参与慢性胃炎发展过程,Hp感染造成患者肠道菌群紊乱,炎性细胞因子水平异常,miR-22和NLRP3蛋白表达异常,这可能与慢性胃炎发病和进展有关。OBJECTIVE To analyze the relationship between Helicobacter pylori(Hp) infection and gastroscopic pathological classifications of chronic gastritis and its influences on intestinal floras, miR-22 and NOD-like receptor protein 3(NLRP3) inflammasomes in gastric mucosal tissues. METHODS A total of 84 patients with chronic gastritis treated in the hospital were enrolled between Jul. 2018 and Jun. 2019. According to presence or absence of Hp infection, they were divided into infection group(n=27) and non-infection group(n=57). The positive rate of Hp in patients with different pathological classifications of chronic gastritis and pathological changes was compared. The number of floras in feces was calculated by logarithmic values. The levels of serum C-reactive protein(CRP) and serum amyloid A(SAA) were detected by immunonephelometry. The level of serum procalcitonin(PCT) was detected by electrochemiluminescence method. The levels of serum interleukin-1(IL-1), interleukin-6(IL-6), interleukin-10(IL-10) and tumor necrosis factor-α(TNF-α) were detected by enzyme-linked immunosorbent assay. The expression levels of miR-22, NLRP3 inflammasomes, apoptosis-associated Speck-like Protein Cotaining(ASC) and cysteine proteinase-1(Caspase-1) in gastric mucosal tissues were detected by Western Bolt. RESULTS There were differences in positive rate of Hp among patients with different pathological classifications, inflammatory degree, inflammatory activity and lamina propria lymphoid follicles(P<0.05). The number of intestinal Bifidobacteria, Escherichia coli and Lactobacillus in infection group was lower than that in non-infection group, while number of Lactobacillus acidophilus was higher than that in non-infection group(P<0.05). The levels of CRP, PCT, SAA and TNF-α in infection group were higher than those in non-infection group, while IL-10 level was lower than that in non-infection group(P<0.05).
