摘　　要：目的探究幽门螺杆菌(Hp)感染对胃癌患者病灶组织中炎性小体核苷酸结合寡聚化结构域样受体蛋白1(NLRP1)和半胱天冬氨酸酶-1(Caspase-1)表达水平的影响。方法选取2018年1月-2020年12月诸暨市中心医院收治的确诊胃癌患者108例为研究对象,根据Hp感染情况分为感染组68例和未感染组40例。分析两组患者临床资料,受试者工作特征(ROC)曲线分析胃癌患者病灶组织中炎性小体NLRP1、Caspase-1表达水平对Hp感染的预测价值。结果感染组患者炎性小体NLRP1、Caspase-1表达水平均高于未感染组患者(P<0.05);肿瘤直径大、肿瘤多发、分期分级程度高、发生血管侵袭和肿瘤转移的感染组患者炎性小体NLRP1和Caspase-1表达水平较高(P<0.05);炎性小体NLRP1、Caspase-1表达水平及联合检测对Hp感染的曲线下面积(AUC)分别为0.820、0.841、0.957。结论炎性小体NLRP1、Caspase-1表达水平与胃癌患者Hp感染相关,临床可通过水平变化来辅助判断Hp感染,进一步预测胃癌的发生与发展。OBJECTIVE To explore the influence of Helicobacter pylori(Hp)infection on expression levels of inflammasomes[nucleotide-binding oligomerization domain-like receptor protein 1(NLRP1),cysteinyl aspartate specific proteinase(Caspase-1)]in lesion tissues of patients with gastric cancer.METHODS Totally 108 patients who were diagnosed with gastric cancer and treated in Zhuji Central Hospital from Jan 2018 to Dec 2020 were recruited as the study objects and were divided into the infection group with 68 cases and the no infection group with 40 cases according to the status of Hp infection.The clinical data of the two groups of patients were analyzed,and the values of the inflammasomes NLRP1 and Caspase-1 in prediction of Hp infection were analyzed by receiver operating characteristic(ROC)curves.RESULTS The expression levels of inflammasomes NLRP1 and Caspase-1 of the infection group were significantly higher than those of the no infection group(P<0.05).In the infection group,the expression levels of inflammasomes NLRP1 and Caspase-1 of the patients with large diameter of tumor,multiple tumors,high staging and grading,vascular invasion and tumor metastasis were relatively high(P<0.05),The areas under curves(AUCs)of the inflammasomes NLRP1,Caspase-1 and their combination were respectively 0.820,0.841 and 0.957 in prediction of the Hp infection.CONCLUSION The expression levels of inflammasomes NLRP1 and Caspase-1 are associated with the Hp infection in the gastric cancer patients,which can be used for auxiliary diagnosis of Hp infection and then further predict the occurrence and development of gastric cancer.
