摘  要:目的分析口腔鳞癌根治术后口腔感染病原菌分布及口腔黏膜Toll样受体4(TLR4)和细胞核因子-κB(NF-κB)表达。方法选择2018年1月-2020年11月在海口市中医医院接受口腔鳞癌根治术的患者169例,根据患者术后是否发生口腔感染分为感染组70例和非感染组99例,另选择同期健康体检者61名为对照组。分析感染患者临床标本检出病原菌分布,检测三组对象口腔黏膜TLR4和NF-κB相对表达量及其对口腔感染的预测价值。结果70例口腔感染患者共检出病原菌112株,其中革兰阴性菌83株,占74.11%,革兰阳性菌29株,占25.89%;三组口腔黏膜TLR4和NF-κB相对表达量比较有统计学差异(P<0.05),其中感染组高于非感染组和对照组,非感染组高于对照组(P<0.05);TLR4和NF-κB两者联合检测对于口腔感染的ROC曲线下面积(AUC)为0.802,敏感度为90.00%,特异度为63.08%。结论口腔鳞癌根治术后口腔感染患者中病原菌分布以革兰阴性菌为主,口腔感染患者存在TLR4和NF-κB相对表达量异常升高,TLR4和NF-κB检测可较好的预测术后口腔感染的发生。OBJECTIVE To investigate the distribution of pathogens isolated from oral squamous cell carcinoma patients who had oral infection after radical surgery and analyze the expression of Toll-like receptor 4(TLR4)/nuclear factor-kappa B(NF-κB)in oral mucosa.METHODS Totally 169 oral squamous cell carcinoma patients who underwent radical surgery in Haikou Hospital of Traditional Chinese Medicine from Jan 2018 to Nov 2020 were enrolled in the study and divided into the infection group with 70 cases and the no infection group with 99 cases according to the status of postoperative oral infection,meanwhile,61 healthy people who received physical examination were chosen as the control group.The distribution of pathogens isolated from clinical specimens of the patients with infection was observed,the relative expression levels of TLR4 and NF-κB in oral mucosa of the three groups of study objects were detected,and the value of the indexes in prediction of oral infection was analyzed.RESULTS Totally 112 strains of pathogens were isolated from 70 patients with oral infection,83(74.11%)of which were gram-negative bacteria,29(25.89%)were gram-positive bacteria.There were significant differences in the relative expression levels of oral mucosa TLR4 and NF-κB among the three groups(P<0.05),the relative expression levels of oral mucosa TLR4 and NF-κB of the infection group were significantly higher than those of the no infection group and the control group,and the relative expression levels of oral mucosa TLR4 and NF-κB of the no infection group were significantly higher than those of the control group(P<0.05).The area under curve(AUC)of the joint detection of TLR4 and NF-κB was 0.802 in prediction of oral infection,with the sensitivity 90.00%,the specificity 63.08%.CONCLUSION The gram-negative bacteria are dominant among the pathogens isolated from the oral squamous cell carcinoma patients with oral infection after radical surgery,the patients with oral infection show abnormal elevation of relative expression levels of TLR4 an
