卵巢癌回肠造口回纳术后切口感染因素分析及对策探讨

摘　　要：目的分析可能引起卵巢癌回肠造口回纳术后切口感染的影响因素.方法回顾性分析2017年1月至2021年2月卵巢癌回肠造口回纳手术297例患者的临床资料,对可能引起切口感染影响因素进行单因素及多因素Logistic回归分析.找出造口回纳术后切口感染的主要影响因素并提出相应的护理对策.结果297例患者术后发生切口感染32例,感染率10.8%;多因素回归分析显示:年龄≥60岁、术前NRS2002评分≥3分、术前化疗疗程未结束、造口周围有皮肤问题是影响卵巢癌回肠造口回纳术后切口独立危险因素(P<0.05);切口留置皮下负压壶是术后切口感染保护性因素(P<0.05).结论卵巢癌回肠造口回纳术后切口感染的危险因素较多,临床上应采取相应的护理对策以降低切口感染率.Objeetive To explore the factors that may cause incision infection after ileostomy reimplantation of ovarian cancer.Methods A total of 297 cases of ovarian cancer patients undergoing ileostomy surgery in Cancer Hospial of The University of Chinese Academy of Sciences(Zhejiang Cancer Hospital)from January 2017 to February 2021 were selected.The relevant clinical data were retrospectively analyzed,and the infuencing factors were analyzed by single factor.The main influencing factors of incision infection were found out according to the analysis results,and the corresponding nursing countermeasures were proposed.Results Among 297 patients,32 cae<(10.8%)of incision infection after operation were developed.Multivariate regression analysis showed that age≥60,preoperative NRS2002≥3 points,preoperative chemotherapy course not finished,and skin problems around the ostomy were independent risk factors for postoperative incision infection after ilcostomy of ovarian cancer.The subcutancous indelling of negative pressure pot was independent protective factor for postoperative incision infection(P<0.05).Conclusion There are many risk factors of incisional infection after ilecostomy reimplantation of ovarian cancer,so comprchensive measures and corresponding nusing counterneasures should be taken to reduce the incisional infection rate.
