早期应用安神合剂联合黛力新治疗老年急性脑梗死后焦虑抑郁临床观察

摘　　要：目的观察早期应用安神合剂联合黛力新对老年急性脑梗死后焦虑抑郁的疗效及神经功能缺损的影响。方法纳入2021-08—2022-06华中科技大学同济医学院附属中西医结合医院收治的老年急性脑梗死后焦虑抑郁共病(PSCAD)患者120例进行前瞻性研究,按随机数字表法分为对照组和研究组,每组又随机分为黛力新组和安神合剂+黛力新组,每组30例,均治疗8周。治疗前后分别运用汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评定焦虑和抑郁,以加拿大人神经病学评分标准(CNS)评定神经功能,通过对比治疗前后评分观察抑郁焦虑症状的评分改善情况及神经功能缺损恢复程度。结果对照组神经功能恢复有效率分别为63.33%和80.00%,研究组分别为80.00%和93.33%;焦虑抑郁症状改善的有效率,对照组分别为63.33%、80.00%,研究组分别为80.00%和93.33%,对照组和研究组比较差异有统计学意义(P<0.001)。治疗后研究组HAMD、HAMA评分明显低于对照组(P<0.001),且安神合剂+黛力新组较黛力新组HAMD、HAMA评分亦下降明显,差异有统计学意义(P<0.001)。结论早期应用安神合剂联合黛力新治疗老年急性脑梗死后抑郁焦虑具有确切的临床疗效,不仅能改善抑郁、焦虑症状,而且还能促进其神经功能恢复,不良反应少,为临床中西医结合治疗老年急性PSCAD开拓了思路。Objective To observe the effects of early application of Anshen mixture combined with Deanxit on anxiety,depression and neurological deficit in elderly patients with acute cerebral infarction.Methods One hundred and twenty aged patients with acute post-stroke complicated anxiety and depression(PSCAD)treated in the Hospital of Integrative Medicine,Tongji Medical College,Huazhong University of Science and Technology from August 2021 to June 2022 were randomly divided into control group and experimental group for prospective study.Each group was randomly divided into Deanxit and Anshen mixture combined with Deanxit,with 30 cases in each group.The treatment of all groups lasted 8 weeks.Before and after treatment,the anxiety and depression of pa⁃tients were used to assess by Hamilton depression scale(HAMD)and Hamilton anxiety scale(HAMA).The neu⁃rologic function were analyzed by Canadian Neurologic scale(CNS).By comparing the score to observe the im⁃provement in depression and anxiety and the degree of CNS recovered.Results The neurologic function recovery in the control and experimental were 63.33%,80.00%,80.00%,93.33%,respectively,and the anxiety and depres⁃sion recovery 63.33%,80.00%,80.00%,93.33%,respectively.The results in experiment group was significantly better than in the control group(P<0.001).After treatment,the scores of HAMD and HAMA in the experimental group were significantly lower than those in the control group,and the scores of HAMD and HAMA in the Anshen mixture combined with Deanxit group were significantly lower than those in the Deanxit group(P<0.001).Conclu⁃sion Anshen mixture combined with Deanxit in the treatment of anxiety and depression after acute cerebral in⁃farction have a definite clinical effect.It can not only improve the symptoms of depression and anxiety,but also promote the recovery of neurologic function,and there are no adverse reactions.So it develop a new way to treat se⁃nile acute PSCAD by integrated traditional Chinese and Western medicine.
