计算机辅助认知训练联合奥拉西坦胶囊治疗卒中后认知障碍的临床观察

摘　　要：目的探讨计算机辅助认知训练联合奥拉西坦胶囊治疗卒中后认知障碍(post-stroke cognitive impairment,PSCI)的临床疗效。方法选取2020年1月至2022年6月在河南省荣军医院收治的94例PSCI患者,按照随机数字表分为观察组与对照组。两组患者给予口服奥拉西坦胶囊、抗血小板聚集、调脂、控制血压等基础治疗。对照组在基础治疗的基础上予传统康复训练,观察组在基础治疗的基础上予计算机辅助认知训练,两组均连续治疗2个月。分别于治疗前、治疗2个月后,比较两组患者的简易精神状态检查量表(MMSE)评分、日常生活能力量表(ADL)评分、洛文斯顿作业疗法认知评定量表(LOTCA)评分、Rivermead行为记忆测验(RBMT)评分及事件相关电位P300的变化。结果治疗前,两组患者MMSE、ADL、LOTCA、RBMT评分及事件相关电位P300差异无统计学意义。治疗后,观察组总有效率93.62%显著高于对照组78.72%;观察组MMSE、ADL、LOTCA、RBMT评分及事件相关电位P300均优于对照组。结论计算机辅助认知训练联合奥拉西坦胶囊治疗PSCI临床疗效确切,在改善患者认知功能、日常生活能力、记忆能力方面临床效果更佳。Objective To explore the clinical effect of computer aided cognitive training combined with Oxiracetam Capsules in the treatment of post-stroke cognitive impairment(PSCI).Methods A total of 94 PSCI patients admitted to Rongjun Hospital of Henan Province from January 2020 to June 2022 were selected and divided into observation group and control group according to random number table.The two groups were given oral Oxiracetam Capsules,anti-platelet aggregation,lipid regulation,blood pressure control and other basic treatments.The control group received traditional rehabilitation training on the basis of basic treatment,and the observation group received computer aided cognitive training on the basis of basic treatment.Both groups received continuous treatment for 2 months.The changes of the Mini-Mental State Examination(MMSE)score,the Ability of Daily Living Scale(ADL)score,the LOTCA score,the Rivermead Behavioral Memory Test(RBMT)score and the event-related potential P300 were compared between the two groups before treatment and 2 months after treatment.Results Before treatment,there were no significant differences in MMSE,ADL,LOTCA,RBMT scores and event-related potential P300 between the two groups.After treatment,the total effective rate of the observation group(93.62%)was significantly higher than that of the control group(78.72%).MMSE,ADL,LOTCA,RBMT scores and event-related potential P300 in the observation group were better than those in the control group.Conclusion Computer aided cognitive training combined with Oxiracetam Capsules has definite clinical efficacy in the treatment of PSCI,and has better clinical effect in improving patients'cognitive function,ability of daily living and memory ability.
