珍龙生肌散联合痔炎冲洗灵治疗肛周脓肿疗效研究

摘　　要：目的:分析珍龙生肌散联合痔炎冲洗灵对肛周脓肿三间隙切开引流术患者术后恢复的影响。方法:选取104例行三间隙切开引流术的肛周脓肿患者,根据非随机临床同期对照研究及患者自愿原则分为观察组、对照组。观察组给予珍龙生肌散外敷、痔炎冲洗灵熏洗,对照组给予凡士林纱条换药、高锰酸钾溶液熏洗。比较两组术后恢复及疼痛情况;比较两组术后21 d中医疗效及术后7、14、21 d创面愈合率;比较两组术后3个月肛门功能。结果:观察组术后3、5、7 d的VAS评分均低于对照组,差异有统计学意义(均P<0.05)。观察组首次排便时VAS评分、住院时间、新生上皮出现时间、创口愈合时间均低于对照组,差异有统计学意义(均P<0.05)。观察组术后7、14、21 d时创面愈合率高于对照组,差异有统计学意义(均P<0.05)。观察组术后21 d中医疗效高于对照组,差异有统计学意义(P<0.05)。观察组术后1个月及3个月Wexner评分低于对照组,差异有统计学意义(均P<0.05)。结论:对肛周脓肿三间隙切开引流术患者给予珍龙生肌散外敷联合痔炎冲洗灵熏洗,能缓解术后疼痛,利于组织创面愈合及肛门功能恢复。Objective:To analyze effects of Zhenlong Shengji powder combined with Zhiyan Chongxiling on postoperative recovery of patients with perianal abscess undergoing three-space incision and drainage.Methods:104 patients with perianal abscess underwent three-space incision and drainage were selected.According to non-randomized clinical concurrent controlled study and principle of voluntariness,the patients were divided into observation group and control group.The observation group were given Zhenlong Shengji powder external application and Zhiyan Chongxiling fumigated.The control group were given vaseline qauze dressing change and potassium permanganate solution fumigated.The postoperative recovery and pain,TCM efficacy at 21 days after surgery,wound healing rate at 7 days,14 days and 21 days after surgery and anal function at 3 months after surgery were compared between the two groups.Results:The VAS scores in the observation group at 3 days,5 days and 7 days after surgery were lower than those in the control group,difference statistically significant(all P<0.05).The VAS score at the first defecation hospital stay new epithelial appearance time and wound healing time in the observation group were lower or shorter than those in the control group,difference statistically significant(all P<0.05).The wound healing rates at 7 days,14 days and 21 days after surgery were higher in the observation group than those in the control group,difference statistically significant(all P<0.05).At 21 days after surgery,the TCM efficacy in the observation group was higher in the control group(P<0.05).The Wexner scores at 1 month and 3 months after surgery were significantly lower in the observation group compared with those in the control group(all P<0.05).Conclusion:External application of Zhenlong Shengji powder combined with Zhiyan Chongxiling fumigation can relieve postoperative pain of patients with perianal abscess undergoing three-space incision and drainage,and conducive to tissue wound healing and anal function recovery.
