摘　　要：目的:探讨miRNA⁃126⁃3p表达与胃癌根治术后肿瘤复发的关系。方法:选取武汉市第三医院2017年2月—2018年2月收治的胃癌患者125例,按是否出现术后肿瘤复发将入组患者分为复发组和未复发组,通过检测患者机体miRNA⁃126⁃3p表达情况,分析其与患者术后复发的关系。结果:125例胃癌患者术后复发39例,复发率为31.20%,其中复发组患者miRNA⁃126⁃3p相对表达量明显低于未复发组患者,差异有统计学意义(P<0.05);Spearman相关性分析显示,miRNA⁃126⁃3p表达水平与胃癌术后复发呈显著负相关(r=-0.621,P<0.05);ROC曲线显示:曲线下面积为0.887,95%置信区间(CI)0.831~0.943,敏感度为0.974,特异度为0.698,最佳截断值为0.59;单因素分析显示:TNM分期、分化程度、联合化疗、肿瘤直径及miRNA⁃126⁃3p表达水平与胃癌术后复发有关(P<0.05);Logistic多因素回归分析:术后未联合化疗、肿瘤TNM分期(Ⅲ期)及分化程度(低分化)、miRNA⁃126⁃3p表达<0.59是胃癌术后复发的危险因素(P<0.05)。结论:胃癌组织中miRNA⁃126⁃3p表达水平是胃癌术后复发的危险因素,与术后复发情况呈显著负相关。Objective:To investigate the relationship between the expression level of miRNA⁃126⁃3p and tumor recurrence after radical gastrectomy.Methods:A total of 125 gastric cancer patients treated in Wuhan Third Hospital from February 2017 to February 2018 were divided into recurrence group and non⁃recurrence group according to whether postoperative tumor recurrence occurred or not.The rela⁃tionship between miRNA⁃126⁃3p expression level and rate of postoperative recurrence was analyzed after detecting the expression level of miRNA⁃126⁃3p of these patients.Results:There were 39 cases of postoperative recurrence occurred in the 125 patients and the recurrence rate was 31.20%.The rela⁃tive expression level of miRNA⁃126⁃3p in the recurrence group was significantly lower than that in the non⁃recurrence group(P<0.05);Spearman correlation analysis showed that the expression level of miRNA⁃126⁃3p was negatively correlated to the rate of postoperative recurrence of gastric cancer(r=-0.621,P<0.05);The ROC curve showed that the area under the curve was 0.887,the 95%confi⁃dence interval(CI)was 0.831⁃0.943,the sensitivity was 0.974,the specificity was 0.698,and the best cut⁃off value of relative miRNA⁃126⁃3p expression was 0.59;Univariate analysis showed that TNM stage,degree of differentiation,combined chemotherapy,tumor diameter,and miRNA⁃126⁃3p expression were related to postoperative recurrence of gastric cancer(P<0.05);Logistic multiple re⁃gression analysis showed the risk factors of postoperative recurrence of gastric cancer included non combination chemotherapy,tumor TNM stage(stageⅢ),degree of differentiation(low differentia⁃tion),and the relative expression level of miRNA⁃126⁃3p<0.59(P<0.05).Conclusion:The expres⁃sion level of miRNA⁃126⁃3p in gastric cancer is negatively correlated to the postoperative recurrence of gastric cancer.Its expression level is of high value in evaluating postoperative recurrence.
