眼周三阳经穴治疗视频终端视疲劳的多中心临床观察

摘　　要：目的观察揿针针刺眼周三阳经穴(双侧攒竹、丝竹空、四白)对视频终端视疲劳(VDT)的作用,优化方案。方法采用多中心、完全随机、盲法、平行组设计、中心分层、区组随机,纳入172例,试验组1(揿针联合玻璃酸钠滴眼液)60例,试验组2(揿针)及对照组(玻璃酸钠滴眼液)各56例,疗程4周,观察泪膜破裂时间(BUT)、泪液分泌试验(SIt)、角膜荧光染色(CFS)、近距离用眼时间及症状体征评分变化。结果(1)试验组1总有效率高于试验组2(P<0.05)及对照组(P<0.01)。(2)试验组1与试验组2比:BUT、SIt、近距离用眼时间、症状体征评分P<0.01,CFS(P<0.05);试验组1与对照组比均P<0.01;试验组2与对照组比:BUT、CFS、SIt、近距离用眼时间P<0.05,症状体征评分P<0.01。(3)视力波动、畏光流泪、眼刺痛、眼红比P<0.01,头晕头痛P<0.05,均有所提升;试验组1、试验组2视力波动较对照组减轻,P<0.01;畏光流泪及眼刺痛:试验组1较试验组2、试验组2较对照组均减轻,P<0.01;试验组2较对照组眼红及头晕头痛好转(P<0.01、P<0.05)。结论眼周三阳经穴揿针能明显改善VDT症状(尤其是眼红、头晕头痛),联合玻璃酸钠滴眼液疗效更佳(尤其是畏光流泪、眼刺痛),值得临床推广应用。Objective To observe the effect of Press needle in sanyang meridians around eyes(bilateral cuanzhu,sizhukong,sibai)of Visual Display Terminal asthenopia(VDT),and enrich its treatment methods.Methods Multicenter,completely random,blinded design,parallel group clinical trial,center layer and block random were designed.172 cases(344eyes)were enrolled,test group 1,60 cases,test group 2,56 cases,control group 56 cases,they were all observated for 4 weeks.Break-up time(BUT),Schirmer test(SIt),Corneal fluorescein staining(CFS),Short-distance use of eyes time and the symptoms of western medicine were observed.Results(1)The total effective rate of test group 1 was 80.0%,which was higher than test group 2(71.4%,P<0.05)and control group(51.8%,P<0.01);(2)compared with test group 2,test group 1 had a significant advantage in BUT,SIt,Short-distance use of eyes time,symptom and sign score(P<0.01),and CFS(P<0.05).Above indexes performed meaningful when test group 1 compared with control group(P<0.01);Compared with control group,test group 2 had an advantage in BUT,CFS,SIt,Short-distance use of eyes time(P<0.05),and symptom and sign score(P<0.01).(3)Visual fluctuations,photophobia and tearing,eye tingling,eye redness(P<0.01)showed meaningful,and so did dizziness and headache(P<0.05);The visual fluctuations of test group 1 and test group 2 were alleviating than control group(P<0.01);In addition,photophobic and tearing and the eye tingling were significantly improved when test group 1 compared with test group 2,and test group 2 compared with control group(P<0.01);Test group 2 was better than control group in white eye hyperemia and dizziness and headache(P<0.01 or P<0.05).Conclusion Press needle in sanyang meridians around eyes(bilateral cuanzhu,sizhukong and sibai)can significantly alleviate VDT symptoms,especially eye redness,dizziness and headache.When it combined with sodium hyaluronate eye drops,the curative effect is much better.
