多模态磁共振成像鉴别诊断乳腺肿块样病变良恶性的价值

摘　　要：目的:探讨多模态磁共振成像(MRI)鉴别诊断乳腺肿块样病变良恶性的价值。方法:回顾性分析河南中医药大学第一附属医院2020年8月至2021年10月期间收治的乳腺肿块样病变患者104例,所有患者均有完整的多模态MRI资料及病理资料。以病理结果为金标准,分析多模态MRI对乳腺良恶性肿块样病变的鉴别诊断价值及和金标准的一致性,并比较乳腺良恶性肿块样病变多模态MRI特征。结果:多模态MRI诊断结果和病理结果的一致性好,κ为0.860;多模态MRI对乳腺肿块样病变的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为93.26%、98.33%、86.36%、90.76%、97.43%;多模态MRI对乳腺恶性肿块样病变中不规则形和毛刺征、不均匀强化和环形强化、流出型时间-信号强度曲线(TIC)、低表观扩散系数(ADC)值的检出率均高于良性肿块,差异均具有统计学意义(P<0.05)。结论:多模态MRI在乳腺肿块样病变的良恶性鉴别中具有较高的诊断价值。Objective To investigate the value of multimodal magnetic resonance imaging(MRI)in the differential diagnosis of benign and malignant breast mass-like lesions.Methods A total of 104 patients with breast lump-like lesions admitted to the First Affiliated Hospital of Henan University of Chinese Medicine from August 2020 to October 2021 were retrospectively analyzed.All patients had complete multimodal MRI data and pathological data.To analyze the value of multimodal MRI in the differential diagnosis of benign and malignant breast mass lesions and the consistency with the gold standard with pathological results as the gold standard,and to compare the multimodal MRI features of benign and malignant breast mass-like lesions.Results The results of multimodal MRI were in good agreement with pathological results(κ=0.860).The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of multimodal MRI for breast lump-like lesions were 93.26%,98.33%,86.36%,90.76%and 97.43%,respectively.The detection rates of irregular shape and burr sign,uneven enhancement and ring enhancement,effusion time-signal intensity curve(TIC)and low apparent diffusion coefficient(ADC)in malignant breast mass lesions were higher than those in the benign breast mass-like lesions,and the differences were statistically significant(P<0.05).Conclusion Multimodal MRI has high diagnostic value in differentiating benign and malignant breast mass lesions.
