普萘洛尔联合595 nm脉冲染料激光治疗婴幼儿血管瘤的临床疗效观察

摘　　要：目的探讨普萘洛尔不同时机联合595 nm脉冲染料激光对婴幼儿血管瘤的瘤体面积、瘤体厚度及血流动力学指标的影响差异。方法选取我院于2017年6月~2021年6月期间收治的婴幼儿混合型血管瘤患儿96例为研究对象,按照治疗方式不同将其分成研究组和对照组,各48例。研究组先给予普萘洛尔口服治疗,再联合595 nm脉冲染料激光治疗;对照组给予普萘洛尔口服和595 nm脉冲染料激光同步联合治疗。对两组患儿血管瘤的瘤体情况(瘤体面积、瘤体厚度)、血流动力学指标(血流峰值和阻力系数)、临床疗效和不良反应进行观察和比较。结果治疗后,两组瘤体面积和瘤体厚度均降低,研究组的瘤体面积和瘤体厚度均小于对照组(P<0.05);治疗后,两组血流峰值和阻力系数均得到改善,研究组的血流峰值和阻力系数均优于对照组(P<0.05);治疗后,研究组的总有效率为95.83%,高于对照组的72.92%;研究组不良反应发生率为6.25%,低于对照组的20.83%(P<0.05)。结论先给予口服普萘洛尔治疗后,再联合595 nm脉冲染料激光治疗治疗婴幼儿血管瘤,能够有效缩小瘤体面积、降低瘤体厚度,改善患儿血流动力学指标,提升临床综合疗效,降低不良反应发生。Objectivs To investigate the effect of propranolol combined with 595 nm pulsed dye laser at different times on tumor area,tumor thickness and hemodynamic indexes of infant hemangioma.Methods From June 2017 to June 2021,96 children with mixed hemangioma of infants and young children who were clinically treated in our hospital from June 2017 to June 2021 were selected as the study subjects,and they were divided into study group and control group according to different treatment methods,with 48 cases in each group.The study group was treated with propranolol orally first,and then with 595 nm pulsed dye laser;The control group was treated with propranolol orally and 595 nm pulsed dye laser synchronously.The tumor body(tumor area,tumor thickness),hemodynamic indexes(peak blood flow and resistance coefficient),clinical efficacy and adverse reactions of hemangiomas in the two groups were observed and compared.Results After treatment,the tumor area and tumor thickness of the two groups decreased.The tumor area and tumor thickness of the study group were smaller than those of the control group(P<0.05);After treatment,the peak blood flow and resistance coefficient of the two groups were improved.The peak blood flow and resistance coefficient of the study group were better than those of the control group,and the difference was significant(P<0.05);After treatment,the total effective rate of the study group was 95.83%,higher than 72.92%of the control group;The incidence of adverse reactions in the study group was 6.25%,which was lower than that in the control group(20.83%)(P<0.05).Conclusion Oral propranolol treatment followed by 595 nm pulsed dye laser treatment can effectively reduce the tumor area,reduce the tumor thickness,improve the hemodynamic indicators of children,enhance the clinical efficacy and reduce adverse reactions.
