摘　　要：目的:研究超声导引下标准化改良塞丁格技术结合心电图(ECG)进行上臂PICC置管的应用效果。方法:选取40例接受上臂PICC置管的病人,按置管方式分为:超声引导下标准化改良塞丁格技术结合ECG置入上臂PICC的观察组与超声引导下标准化改良塞丁格技术不结合ECG置入上臂PICC的对照组,在置管7d后对比两组患者的置管成功率、穿刺部位渗血、穿刺点异常、手指肿胀、机械性静脉炎等情况。结果:观察组患者置管成功率明显高于对照组,置管7天后,观察组穿刺部位渗血、穿刺点异常、手指肿胀、机械性静脉炎等与对照组相比均明显更优(P<0.05)。结论:超声导引下标准化改良塞丁格技术结合ECG进行上臂PICC置管的成功率更高,能降低感染和静脉炎的发生。Objective: To study the application effect of ultrasonic guided standardized modified Seidinger technique combined with electrocardiogram(ECG) in PICC catheterization in upper arm. Methods: 40 patients receiving upper arm PICC catheterization were selected and divided into: the observation group received ultrasound guided standardized modified Seidinger technology combined with ECG in PICC catheterization in upper arm and the control group received ultrasound guided modified Seidinger technology without combination with ECG in PICC catheterization in upper arm. The catheterization success rate, blood oozing at the puncture point, puncture point anomalies, finger swelling,mechanical phlebitis, and so on of the two groups of patients after 7 d of insertion were compared. Results: The catheterization success rate in the observation group was significantly higher than that in the control group. After 7 days of catheterization, blood oozing at the puncture point, puncture point anomalies, finger swelling and mechanical phlebitis in the observation group were significantly better than those in the control group(P<0.05). Conclusion: Ultrasound guided standardized modified Seidinger technique combined with ECG has a higher success rate of PICC catheterization in the upper arm, and can reduce the incidence of infection and phlebitis.
